摘要
目的 分析鼻咽癌(NPC)患者放疗后发生放射性口腔黏膜炎(ROM)的影响因素。方法 将放疗后发生ROM的NPC患者86例作为发生组,并以1∶1配比选取同期放疗后未发生ROM的NPC患者86例作为未发生组,采用自制调查表收集2组性别、年龄、吸烟史、文化水平、口腔卫生情况、服用抗生素等资料,分析NPC患者放疗后发生ROM的影响因素。结果 BMI指数、有无吸烟史、饮酒史、化疗史、口腔卫生情况、口腔PH值、有无服用抗生素、使用口腔黏膜保护剂是影响NPC患者放疗后发生ROM的单因素(P<0.05);logistic回归分析显示,BMI≥24 kg/m^(2)、有吸烟史、有饮酒史、有化疗史、口腔卫生较差、口腔PH值≤7.0是NPC患者放疗后发生ROM的危险因素,服用抗生素、使用口腔黏膜保护剂是NPC患者放疗后发生ROM的保护因素(P<0.05)。结论 NPC患者放疗后发生ROM的影响因素较多,吸烟、饮酒、肥胖、口腔卫生不理想等均可对放疗后发生ROM产生不利影响,临床应及早针对这些危险因素采取干预措施,减少ROM的发生,改善预后。
Objective To analyze the influencing factors of radioactive oral mucositis(ROM) after radiotherapy in patients with nasopharyngeal carcinoma(NPC).Methods We collected 86 NPC patients after radiotherapy,and selected 86 NPC patients without ROM after radiotherapy with 1∶1 ratio.The gender,age,smoking history,cultural level,oral hygiene,antibiotics and analyzed the influencing factors of NPC patients after radiotherapy.Results BMI index,history of smoking,alcohol history,chemotherapy,oral hygiene,oral PH,availability of antibiotics,use of oral mucosal protective agents were single factors affecting the development of ROM after radiotherapy in patients with NPC(P<0.05);The logistic regression analysis showed that,BMI≥24 kg/m^(2),has a history of smoking,alcohol consumption,chemotherapy,poor oral hygiene,and oral PH ≤7.0 are risk factors for ROM in NPC patients after radiotherapy,Antibiotics with antibiotics and oral mucosal protectants are protective factors for the development of ROM after radiation in patients with NPC(P<0.05).Conclusion NPC patients have more factors affecting ROM after radiotherapy,and smoking,alcohol consumption,obesity and unsatisfactory oral hygiene can have adverse effects on ROM after radiotherapy.Interventions should be taken for these risk factors early in clinical practice to reduce the occurrence of ROM and improve prognosis.
