摘要
目的分析沙丁胺醇联合布地奈德雾化吸入对慢性阻塞性肺疾病急性加重期(AECOPD)患者气道重塑、炎症介质水平的影响。方法选择本院2019年1月至2021年6月接收的100例AECOPD患者,按照治疗方案将其分为对照组和观察组,各50例。对照组采取布地奈德雾化吸入治疗,观察组采取沙丁胺醇联合布地奈德雾化吸入治疗。比较两组的血气、肺功能及气道重塑指标、炎症介质水平、生存质量评分。结果治疗后,两组的PaCO_(2)均降低,PaO_(2)、SaO_(2)、pH、HCO_(3)-水平均升高,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的SVC、FEV1/FVC、PEF、DLco均较治疗前升高,且观察组高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的T/D、WA%、MMP-9、TGF-β_(1)水平均较治疗前降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的PCT、IL-8、TNF-α、sTREM-1水平均低于治疗前,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的SGRQ评分均降低,QOL评分均升高,且观察组优于对照组,差异具有统计学意义(P<0.05)。结论沙丁胺醇联合布地奈德雾化吸入可明显改善AECOPD患者的血气、肺功能指标,减轻气道重塑及炎症反应,提高生存质量,值得临床推广应用。
Objective To analyze the effects of aerosol inhalation of salbutamol combined with budesonide on airway remodeling and inflammatory mediators levels in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods One hundred patients with AECOPD enrolled in our hospital from January 2019 to June 2021 were selected and divided into control group and observation group by treatment plan,with 50 cases in each group.The control group was treated with budesonide aerosol inhalation,and the observation group was treated with aerosol inhalation of salbutamol combined with budesonide.The blood gas,lung function,airway remodeling indexes,inflammatory mediators levels and the score of quality of life were compared between the two groups.Results After treatment,PaCO_(2) in the two groups decreased,PaO_(2),SaO_(2),pH and HCO_(3)-levels increased,those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).After treatment,SVC,FEV1/FVC,PEF and DLco in the two groups were higher than those before treatment,those in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).After treatment,T/D,WA%,MMP-9 and TGF-β_(1) levels in the two groups were lower than those before treatment,those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of PCT,IL-8,TNF-αand sTREM-1 in the two groups were lower than those before treatment,those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).After treatment,SGRQ score in the two groups decreased,QOL score increased,those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).Conclusion The aerosol inhalation of salbutamol combined with budesonide can significantly improve the blood gas and pulmonary function indexes of AECOPD
