摘要
目的:观察乙肝肝硬化患者接受微生态制剂与恩替卡韦联合使用的治疗效果。方法:将2018年1月-2020年12月某院收治的患有乙肝肝硬化的92例患者随机分为观察组和对照组各46例,两组患者均予恩替卡韦口服治疗,观察组同时予以微生态制剂使用,持续治疗6个月后比较两组患者的乙肝病毒及腹部彩超检查结果、肝相关指标、肠道菌群。结果:治疗后,观察组脾脏厚度、门静脉直径小于对照组,乙肝病毒DNA转阴率高于对照组(P<0.05);观察组患者的天冬氨酸氨基转移酶、丙氨酸氨基转移酶、总胆红素水平均低于对照组(P<0.05);观察组患者的透明质酸、层粘连蛋白、III型前胶原水平均低于对照组(P<0.05);观察组的双歧杆菌、乳酸杆菌数量均高于对照组,肠杆菌、肠球菌数量均低于对照组(P<0.05)。结论:微生态制剂联合恩替卡韦使用,可帮助乙肝肝硬化患者保护肝功能、延缓肝脏纤维化进程,提高乙肝病毒转阴率。
Objective:To explore the curative effect of probiotics combined with entecavir on hepatitis B cirrhosis.Methods:A total of 92 patients with hepatitis B cirrhosis admitted to a hospital from January 2018 to December 2020 were randomly divided into observation group and control group,with 46 cases in each group.Both groups were treated with oral entecavir.On this basis,observation group was treated with probiotics.After 6 months of continuous treatment,examination results of hepatitis B virus and abdominal color Doppler ultrasound,liver related indexes and intestinal floras were compared between the two groups.Results:After treatment,spleen thickness and portal vein diameter in observation group were lower than those in control group,and negative conversion ratio of hepatitis B virus(HBV)DNA was higher in observation group(P<0.05).The levels of aspartate aminotransferase,alanine aminotransferase and total bilirubin were lower in observation group than in control group(P<0.05),and levels of hyaluronic acid,laminin and type III procollagen were lower in observation group(P<0.05).The counts of bifidobacteria and lactobacilli in observation group were more than those in control group,while counts of enterobacteria and enterococci were lower in observation group(P<0.05).Conclusion:Probiotics combined with entecavir can protect liver function,delay the process of liver fibrosis and increase negative conversion ratio of hepatitis B virus in patients with hepatitis B cirrhosis.
