摘要
目的探讨透析用留置针(以下简称留置针)在老年透析患者中的应用及效果分析。方法选择2019年9月—2020年1月在南京医科大学附属江宁医院血液透析的老年患者46例,以穿刺方式进行分组,钢针组实施钢针穿刺,留置针组使用留置针,比较两组渗血/血肿发生情况、硬结/瘢痕发生情况、压力报警发生情况、舒适度、止血时间。结果不同穿刺方式行穿刺后,留置针组的渗血/血肿发生率为8.69%,明显低于钢针组的43.47%,差异有统计学意义(χ^(2)=5.216,P=0.019);留置针组的硬结/瘢痕发生率为4.34%,明显低于钢针组的34.78%,差异有统计学意义(χ^(2)=4.973,P=0.026);留置针组的压力报警发生率为0.00%,明显低于钢针组的34.78%,差异有统计学意义(χ^(2)=7.415,P=0.006);留置针组的止血时间为(23.21±0.87)min,短于钢针组的(24.62±1.14)min,差异有统计学意义(t=4.715,P<0.001);留置针组的使用留置针、钢针过程舒适度评分为(82.43±5.39)分,明显高于钢针组的(70.37±4.71)分,差异有统计学意义(t=8.080,P<0.001)。结论透析用留置针在老年患者中的使用,减少了穿刺对血管及周围组织的损伤,提高了患者透析中的舒适度,减少了止血时间,降低了老年患者维护血管通路的难度及护士后期穿刺的难度。该次研究发现,留置针的使用,对血管长度有一定要求,留置针软管处无管夹,连接不简便,设计上有待改善。
Objective To explore the application and effect analysis of indwelling needles for dialysis(hereinafter referred to as indwelling needles)in elderly dialysis patients.Methods 46 elderly patients who underwent hemodialysis in Jiangning Hospital Affiliated to Nanjing Medical University from September 2019 to January 2020 were selected and grouped by puncture,the steel needle group was subjected to steel needle puncture,and the indwelling needle group used indwelling needles.The oozing/hematoma,induration/scar,pressure alarm,comfort,hemostasis time of the two groups were compared.Results After puncturing with different puncture methods,the incidence of hemorrhage/hematoma in the indwelling needle group was 8.69%,which was significantly lower than the 43.47%in the steel needle group,the difference was statistically significant(χ^(2)=5.216,P=0.019).The incidence of induration/scarring in the indwelling needle group was 4.34%,which was significantly lower than the 34.78%in the steel needle group,the difference was statistically significant(χ^(2)=4.973,P=0.026).The pressure alarm rate of the indwelling needle group was 0.00%,which was significantly lower than the 34.78%of the steel needle group,the difference was statistically significant(χ^(2)=7.415,P=0.006).The hemostasis time of the indwelling needle group was(23.21±0.87)min,which was shorter than that of the steel needle group(24.62±1.14)min,the difference was statistically significant(t=4.715,P<0.001).The comfort score of the indwelling needle and steel needle in the indwelling needle group was(82.43±5.39)points,which was significantly higher than the(70.37±4.71)points in the steel needle group,the difference was statistically significant(t=8.080,P<0.001).Conclusion The use of indwelling needles for dialysis in elderly patients reduces the damage to blood vessels and surrounding tissues caused by puncture,improves the comfort of patients during dialysis,reduces the time of hemostasis,and reduces the difficulty of maintaining vascular access for elderly patie
