摘要
目的分析糖尿病患者幽门螺杆菌(HP)阳性临床特点。方法选取该院2021年1—12月期间100例糖尿病患者为研究对象,以其是否为HP阳性分为对照组(61例,单纯糖尿病)、观察组(39例,糖尿病合并HP阳性),比较两组血糖水平[空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)]、胃镜检查结果(反流性食管炎、溃疡大小、胃炎、消化道溃疡、胃炎类型、胃肠息肉)。结果观察组FPG、2 hPG、HbA1c、HOMA-IR水平均较对照组高,差异有统计学意义(P<0.05);观察组反流性食管炎、胃炎、消化道溃疡、胃肠息肉检出率为12.82%、38.46%、33.33%、15.38%,均较对照组1.64%、13.11%、9.84%、3.28%高,差异有统计学意义(P<0.05);胃炎类型中,观察组糜烂性胃炎检出率为60.00%,较对照组12.50%高,差异有统计学意义(P<0.05);消化道溃疡中,观察组溃疡直径≥1.0 cm病灶检出率为78.95%,较对照组37.50%高,差异有统计学意义(P<0.05)。结论糖尿病患者合并HP后,其血糖水平明显升高,胰岛素抵抗程度增加,同时会增加患者消化道疾病、黏膜溃疡发生率。
Objective To analyze the clinical characteristics of Helicobacter pylori(HP)positive in diabetic patients.Methods 100 diabetic patients in the hospital from January to December 2021 were selected as the research objects,and they were divided into control group(61 cases,pure diabetes)and observation group(39 cases,diabetes combined with HP positive)according to whether they were HP positive or not.The blood glucose levels[fasting blood glucose(FPG)and 2 h postprandial blood glucose(2 hPG),Glycated hemoglobin(HbA1c),insulin resistance index(HOMA-IR)],gastroscopy results(reflux esophagitis,ulcer size,esophagitis,peptic ulcers,gastritis type,gastrointestinal polyps)were compared between the two groups.Results The levels of FPG,2 hPG,HbA1c and HOMA-IR in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The detection rates of reflux esophagitis,gastritis,peptic ulcer and gastrointestinal polyps in the observation group were 12.82%,38.46%,33.33%and 15.38%.All were higher than the control group by 1.64%,13.11%,9.84%and 3.28%,the difference was statistically significant(P<0.05).Among the types of gastritis,the detection rate of erosive gastritis in the observation group was 60.00%,which was higher than that in the control group 12.50%,the difference was statistically significant(P<0.05).Among peptic ulcers,the detection rate of lesions with a diameter of≥1.0 cm in the observation group was 78.95%,which was higher than that in the control group 37.50%,the difference was statistically significant(P<0.05).Conclusion The blood glucose level and the degree of insulin resistance increase significantly in diabetic patients combined with HP.At the same time,it will increase the incidence of gastrointestinal diseases and mucosal ulcers in patients.
