肾消方对糖尿病肾病大鼠肾脏损伤的干预作用 被引量:1

Intervention Effect of Shenxiao Recipe on Kidney Injury in Rats with Diabetic Nephropathy

摘要 目的:探究肾消方对糖尿病肾病(diabetic nephropathy,DN)大鼠的肾脏保护作用。方法:90只雄性SD大鼠随机选10只为正常组,其余80只大鼠采用高脂高糖饲料饲喂+腹腔注射小剂量链脲佐菌素(streptozocin,STZ)建立DN模型。将成模大鼠随机分为模型组、厄贝沙坦组(14.12 g·kg^(-1))及肾消方低(5.63 g·kg^(-1))、中(11.26 g·kg^(-1))、高(22.52 g·kg^(-1))剂量组。肾消方给药组灌胃给予相应剂量的肾消方溶液,厄贝沙坦组给予厄贝沙坦溶液,模型组和正常组灌胃给予等体积的纯净水,每天1次,连续20周。采用全自动大型生化分析仪检测大鼠血清总蛋白(total protein,TP)、血肌酐(serum creatinine,Scr)、血清白蛋白(albumin,ALB)、尿酸(uric acid,UA)、尿素氮(urea nitrogen,BUN)及尿肌酐(urine creatinine,Ucr)的水平,计算内生肌酐清除率(creatinine clearance rate,Ccr);代谢笼留取大鼠24 h尿液,终点法检测24 h尿蛋白(urine total protein,UTP)的水平;光镜及透射电镜下观察大鼠肾脏的病理变化,测定大鼠肾小球面积,并计算肾小球体积(glomerular volume,GV)。结果:与正常组比较,模型组Scr、BUN、UA、Ccr、GV的水平及4周、8周、12周、16周、20周时的24 hUTP水平显著升高(P<0.01);与模型组比较,肾消方低剂量组BUN及UA的水平显著降低(P<0.05);肾消方高剂量组UA的水平显著降低(P<0.05);肾消方低、中、高剂量组Ccr、GV水平及4周、8周、12周、16周、20周时的24 hUTP水平显著降低(P<0.05)。病理检查发现,正常组大鼠肾脏结构无异常,肾小球囊腔宽大,系膜基质分布正常,足突整齐分布;模型组大鼠肾小球肥大且囊腔狭窄,球囊粘连,系膜基质分布增多,系膜区宽度增加,基底膜厚度增加,足突重度融合,肾小管上皮细胞肿胀,肾间质部分纤维化;与模型组比较,各给药组大鼠的病理变化得到不同程度的改善。结论:肾消方可明显改善对糖尿病肾病瘀血阻络证大鼠的蛋白尿排泄,具� Objective:To explore the renal protective effect of Shenxiao Recipe on diabetic nephropathy(DN)rats.Methods:10 of 90 male SD rats were randomly selected as normal group,and the remaining 80 rats were fed with high-fat and high-sugar diet+intraperitoneal injection of low-dose streptozocin(STZ)to establish DN model.The model rats were randomly divided into model group,irbesartan group(14.12 g·kg^(-1)),Shenxiaofang low(5.63 g·kg^(-1)),medium(11.26 g·kg^(-1)),high(22.52 g·kg^(-1))dose group.The Shenxiaofang administration group was given the corresponding dose of Shenxiaofang solution by gavage,the irbesartan group was given irbesartan solution,and the model group and the normal group were given the same volume of purified water by gavage,once a day for 20 consecutive weeks.An automatic large-scale biochemical analyzer was used to detect rat serum total protein(TP),serum creatinine(Scr),serum albumin(ALB),uric acid(UA),urea nitrogen(urea).The levels of nitrogen,(BUN)and urine creatinine(Ucr)were used to calculate the endogenous creatinine clearance rate(Ccr);24-hour urine was collected from the rats in the metabolic cage,and the 24-hour urine protein(Urine total total)was detected by the end-point method.The pathological changes of the rats were observed under light microscope and transmission electron microscope,the glomerular area of the rats was measured,and the glomerular volume(GV)was calculated.RESULTS:Compared with the normal group,the levels of Scr,BUN,UA,Ccr,GV and the 24 h urinary protein levels at 4 W,8 W,12 W,16 W,and 20 W in the model group were significantly increased(P<0.01);In comparison,the levels of BUN and UA in the low-dose Shenxiao Recipe group were significantly decreased(P<0.05);the levels of UA in the high-dose Shenxiao Recipe group were significantly decreased(P<0.05);The levels of Ccr,GV and 24 h urine protein levels of rats in the dose group at 4 W,8 W,12 W,16 W,20 W were significantly decreased(P<0.05).Pathological examination showed that the renal structure of the rats in the normal g
