摘要
目的:系统评价针灸结合言语康复训练治疗卒中后构音障碍的临床疗效。方法:检索建库至2021年1月中国期刊全文数据库(CNKI)、万方数据资源系统(WF)、维普数据库(VIP)、中国生物医学文献数据库(CBM)、PubMed、Embase、Cochrane Library数据库中针灸结合言语康复训练治疗卒中后构音障碍临床随机对照试验文献,对纳入文献进行资料提取和质量评价,使用RevMan5.4软件进行Meta分析。结果:共纳入30篇文献,病例数2891例。临床疗效资料合并:异质性检验P=0.03,I^(2)=36%,RR=1.34,95%可信区间[1.28,1.40],效应检验值Z=13.16,P<0.01,表明ACWSRT组有效率显著优于SRTA组。改良Frenchay构音障碍量表症状评分资料合并:异质性检验,P<0.01,I^(2)=88%,MD=3.91,95%CI[3.20,4.62],效应检验值Z=10.84,P<0.01,表明ACWSRT组有效率显著优于SRTA组。语音清晰度资料合并:异质性检验P=0.89,I^(2)=0%,MD=4.68,95%CI[-0.89,10.26],效应检验值Z=1.65,P=0.1,表明ACWSRT组和SRTA组之间无统计学差异。结论:针灸结合言语康复训练的临床疗效明显优于单纯言语康复训练组,针灸结合言语康复训练治疗卒中后构音障碍疗效确切且安全性高,但仍需高质量大样本多中心随机对照试验提供进一步的证据。
Objective:To systematically evaluate the clinical efficacy of acupuncture combined with speech rehabilitation training in the treatment of post-stroke dysarthria.Methods:The database was searched to January 2021 in China National Journal Full-text Database(CNKI),Wanfang Data Resource System(WF),VIP Database(VIP),China Biomedical Literature Database(CBM),PubMed,Embase,and Cochrane Library databases.The clinical randomized controlled trials of acupuncture combined with speech rehabilitation training for post-stroke dysarthria were included in the data extraction and quality evaluation,and RevMan5.4 software was used for meta-analysis.RESULTS:A total of 30 articles were included,with 2891 cases.Combined clinical efficacy data:heterogeneity test P=0.03,I^(2)=36%,RR=1.34,95% confidence interval[1.28,1.40],effect test value Z=13.16,P<0.01,indicating that the ACWSRT group has a significant effective rate better than the SRTA group.Modified French dysarthria scale symptom score data pooling:heterogeneity test,P<0.01,I^(2)=88%,MD=3.91,95%CI[3.20,4.62],effect test value Z=10.84,P<0.01,It showed that the effective rate of ACWSRT group was significantly better than that of SRTA group.Speech intelligibility data pooled:heterogeneity test P=0.89,I^(2)=0%,MD=4.68,95%CI[-0.89,10.26],effect test value Z=1.65,P=0.1,indicating that ACWSRT group and SRTA There was no statistical difference between the groups.Conclusion:The clinical efficacy of acupuncture combined with speech rehabilitation training is significantly better than that of the simple speech rehabilitation training group.Acupuncture combined with speech rehabilitation training is effective and safe in the treatment of post-stroke dysarthria,but high-quality large-sample multi-center randomized controlled trials are still needed.further evidence.
