摘要
目的:探讨超声心动图检查联合血浆脑钠肽(BNP)检测在慢性肺源性心脏病(CPHD)患者右心室功能评估中的应用价值。方法:将2019年8月至2020年8月江苏省射阳县人民医院收治的50例CPHD患者设为观察组,将同期在该医院进行体检的50例健康人设为对照组。对两组研究对象均进行超声心动图检查及血浆BNP检测,并对其检查/检测结果进行比较。结果:对两组研究对象进行超声心动图检查的结果显示,观察组患者的右心房横径(RAD)、右心室前后径(RVD)和肺动脉主干内径(PMAD)均大于对照组健康人,肺动脉收缩压(PASP)和Tei指数均高于对照组健康人,三尖瓣环收缩期位移(TAPSE)、右室面积变化率(RVFAC)和三尖瓣环收缩期峰值速度(s’)均低于对照组健康人,差异有统计学意义(P<0.05)。观察组患者血浆BNP的平均水平为(923.54±33.60)pg/mL,对照组健康人血浆BNP的平均水平为(80.52±12.87)pg/mL,二者相比差异有统计学意义(P<0.05)。结论:CPHD患者的超声心动图表现和血浆BNP的水平与健康人相比差异较大,超声心动图检查联合血浆BNP检测在此病患者右心室功能的评估中具有较高的应用价值。
Objective:To investigate the application value of echocardiography combined with plasma brain natriuretic peptide(BNP)in the evaluation of right ventricular function in patients with chronic pulmonary heart disease(CPHD).Methods:A total of 50 CPHD patients admitted to Sheyang People’s Hospital of Jiangsu Province from August 2019 to August 2020 were enrolled as the observation group,and 50 healthy subjects who underwent physical examination in this hospital during the same period were enrolled as the control group.Echocardiography and plasma BNP were performed in both groups,and the results were compared.Results:The results of echocardiography showed that the right atrial transverse diameter(RAD),right ventricular anterior and posterior diameter(RVD)and pulmonary artery main diameter(PMAD)of patients in the observation group were larger than those in the control group,and the pulmonary artery systolic pressure(PASP)and Tei index were higher than those in the control group.The tricuspid annulus systolic displacement(TAPSE),rate of change in right ventricular area(RVFAC)and peak rate of tricuspid annulus systolic velocity(S‘)were lower than those in the control group,and the differences were statistically significant(P<0.05).The average plasma BNP level was(923.54±33.60)pg/mL in the observation group and(80.52±12.87)pg/mL in the control group,and the difference was statistically significant(P<0.05).Conclusion:Echocardiography and plasma BNP levels in CPHD patients are significantly different from those in healthy subjects.Echocardiography combined with plasma BNP has high application value in the evaluation of right ventr icular function in CPHD patients.
