摘要
目的观察茵陈蒿汤对梗阻性黄疸肝胆湿热证术后IL-6及免疫功能的影响,探讨梗阻性黄疸术后更加合理的治疗方案。方法采用随机数表法将2020年1月至2021年9月福建中医药大学附属人民医院普外科收治的60例梗阻性黄疸肝胆湿热证术后患者分为两组,每组各30例,对照组予以常规治疗,观察组在常规治疗的基础上辅以茵陈蒿汤治疗,每组均于术后第3天、第7天、第10天检测血液中的炎症因子IL-6及免疫功能指标T细胞亚群CD4^(+)T细胞、CD8^(+)T细胞及其比值。结果两组在茵陈蒿汤干预前,即术后第3天IL-6,CD4^(+)T细胞,CD8^(+)T淋巴细胞比较,差异无统计学意义(P>0.05);两组在茵陈蒿汤干预后,即术后第7,10天IL-6,CD4^(+)T细胞,CD8^(+)T细胞即CD4^(+)/CD8^(+)比较,差异有统计学意义(P<0.05)。结论茵陈蒿汤能够明显降低梗阻性黄疸肝胆湿热证术后的炎症因子水平,提高免疫功能T淋巴细胞亚群水平,调控作用确切,值得应用于临床。
Objective To observe the effect of Yinchenhao Decoction on IL-6 and immune function after operation on obstructive jaundice with liver-gallbladder damp-heat syndrome,and to explore a more reasonable postoperative treatment scheme for obstructive jaundice.Methods A total of 60 patients who had undergone operation on obstructive jaundice with liver-gallbladder damp-heat syndrome in the Department of General Surgery of our hospital from January 2020 to September 2021 were randomly divided into two groups,with 30 cases in each group.The control group was treated with routine treatment,and the observation group was treated with Yinchenhao Decoction on the basis of routine treatment.The inflammatory factor,interleukin-6(IL-6)and immune function indexes,T cell subsets CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)Tcell ratio were detected on the 3 rd,7th and 10 th day after operation.Results There was no significant difference in IL-6,CD4^(+)T cells and CD8^(+)T cells between the two groups before Yinchenhao Decoction intervention,that is,on the third day after operation(P>0.05);There was significant difference in IL-6,CD4^(+)T cells and CD8^(+)T cells(CD4^(+)/CD8^(+))between the two groups after Yinchenhao Decoction intervention,i.e.7 and 10 days after operation(P<0.05).Conclusion Yinchenhao Decoction can significantly reduce the level of inflammatory factors and improve the level of T cell subsets suggesting immune function after operation on obstructive jaundice with liver-gallbladder damp-heat syndrome.It has a definite regulatory effect and is worthy of clinical application.
