颈动脉狭窄患者临床诊断中磁共振诊断的应用及影像学特点探讨

摘要 目的:探讨颈动脉狭窄患者临床诊断中磁共振诊断的应用及影像学特点。方法:选取2020年1月—2020年10月驻马店市中心医院收治的颈动脉狭窄患者82例作为研究对象,所有患者均进行数字减影血管造影检查(DSA)与磁共振检查(MRI)。比较两种检查方法检查阳性率、颈动脉狭窄情况、准确性与分析磁共振检查影像学特点。结果:MRI检查与DSA检查阳性率相比,差异无统计学意义(χ^(2)=1.324,P>0.05)。MRI检查与DSA检查颈动脉狭窄情况相比,差异无统计学意义(χ^(2)=1.512,P>0.05)。MRI检查与DSA检查准确性相比,差异无统计学意义(χ^(2)=1.324,P>0.05)。82例患者MRI图像完整,存在血管狭窄154支,4级5支、3级11支、2级16支、1级50支,狭窄血管存在斑块内出血29支,TlWI序列:高信号。T2WI序列:等信号。TOF序列:高信号。25支狭窄血管存在破裂纤维帽,各序列上显示斑块表面形态不一,存在龛影。结论:颈动脉狭窄临床诊断中,使用磁共振检查可以对患者的颈动脉狭窄情况进行明确,进而可以为临床诊断与治疗提供科学的依据。 Objective: To investigate the application and imaging characteristics of magnetic resonance imaging in the clinical diagnosis of carotid artery stenosis. Methods: A total of 82 patients with carotid artery stenosis who were admitted to the hospital from January 2020 to October 2020 were selected as the research subjects. All patients underwent digital subtraction angiography(DSA) and magnetic resonance imaging(MRI). The positive rate, carotid artery stenosis, accuracy and imaging characteristics of MRI were compared between the two methods. Results: There was no statistically significant difference in the positive rate between MRI and DSA(χ^(2)=1.324, P>0.05). There was no statistically significant difference between MRI examination and DSA examination of carotid artery stenosis(χ^(2)=1.512, P>0.05). There was no statistically significant difference in the accuracy of MRI examination and DSA examination(χ^(2)=1.324, P>0.05). The MRI images of 82 patients were complete, and there were 154 vessels with stenosis, including 5 vessels of grade 4, 11 vessels of grade 3, 16 vessels of grade 2, and 50 vessels of grade 1. There were 29 intraplaque hemorrhages in the stenotic vessels, TlWI sequence: high signal, T2WI sequence: isointense, TOF sequence: high signal.There were ruptured fibrous caps in 25 stenotic vessels, and the plaque surface morphology was different on each sequence, and there were niche shadows. Conclusion: In the clinical diagnosis of carotid artery stenosis, the use of magnetic resonance examination can clarify the carotid artery stenosis of patients, and then provide a scientific basis for clinical diagnosis and treatment.
