摘要
目的 探究高频超声在无症状高尿酸血症(HUA)检测中的应用价值。方法 选择2020年1~12月牡丹江医学院附属红旗医院收治的无症状高尿酸血症患者共300例作为观察组,并选择同时间的72例痛风性关节炎(GA)患者为对照组。对比患者的超声表现、病变关节、血尿酸平均值、关节病变发生率及踝、膝、第一跖趾关节具体情况等。结果 观察组中关节共1800个,其中病变关节180个(10.00%)。28个关节见侵蚀(13.33%),54个关节检出痛风石(25.71%);观察组患者的血尿酸水平低于对照组,且关节病变数180个(10.00%)明显低于对照组的210个(70.00%)(P<0.05);观察组患者中的第一跖趾关节病变数90个(50.00%)稍低于对照组的92个(43.81%)(P>0.05),两组患者的踝关节和膝关节比较,差异无统计学意义(P>0.05)。结论 针对高尿酸血症患者,通过采用高频超声进行诊断检测,发现关节病变存在特异性超声表现,能够很好地明确患者的病变类型、程度和主要部位,且在早期时便能够发现无症状高尿酸血症患者的关节病变,适合应用于临床推广,有一定应用价值。
Objective To investigate the application value of high-frequency ultrasound in the detection of asymptomatic hyperuricemia(HUA). Methods A total of 300 patients with asymptomatic HUA admitted to Hongqi Hospital Affiliated to Mudanjiang Medical University from January to December 2020 were selected as the observation group,and 72 patients with gouty arthritis(GA) during the same period were selected as the control group. The ultrasonic manifestations, diseased joints, mean levels of blood uric acid, incidence of joint lesions, and specific conditions of ankle, knee and first metatarsophalangeal joint were compared between the two groups. Results There were 180(10.00%)diseased joints in 1800 joints in the observation group. 28 joints were seen to be eroded(13.33%), and gout stones were detected in 54 joints(25.71%). The blood uric acid levels of the observation group were lower than those of the control group, and the incidence of joint lesions of the observation group(180 joints, 10.00%) was significantly lower than that of the control group(210 joints, 70.00%)(P<0.05). The incidence of the first metatarsophalangeal joint pain of the observation group(90 joints, 50.00%) was slightly lower than that of the control group(92 joints, 43.81%)(P>0.05), and there were no differences between the two groups in the conditions of ankle and knee joints(P >0.05). Conclusion High-frequency ultrasound detection reveals the existence of specific ultrasonic manifestations of joint lesions in patients with HUA, which can well clarify the type, severity and main location of lesions, and can detect joint lesions in patients with AH at an early stage. Therefore, it is worthy of clinical promotion and application.
