摘要
目的:观察穴位埋线治疗偏头痛的临床疗效。方法:选取2020年7月至2021年7月于上海市第五人民医院中医科、针灸科门诊就诊的偏头痛患者60例,按照随机数字表法分为埋线组和针刺组,每组30例,最终完成病例数共计58例,针刺组脱落2例。埋线组给予穴位埋线治疗,针刺组给予普通针刺治疗。比较两组患者的临床疗效及治疗前后及治疗后1周中医证候积分、疼痛视觉模拟量表(visual analogue score,VAS)评分、脑血管椎动脉及基底动脉收缩期平均血流速度变化情况。结果:埋线组治疗后及治疗后1周中医证候积分均低于针刺组,差异有统计学意义(P<0.05);埋线组治疗后及治疗后1周VAS评分均低于针刺组,差异有统计学意义(P<0.05);埋线组治疗后及治疗后1周椎动脉、基底动脉平均血流速度均低于针刺组,差异有统计学意义(P<0.05);埋线组有效率为93.3%,针刺组有效率为85.7%,两组有效率比较,差异具有统计学意义(P<0.05);两组不良事件发生率比较,差异无统计学意义(P>0.05)。结论:穴位埋线和针刺治疗均可改善偏头痛患者临床症状,降低基底动脉和椎动脉平均血流速度,且穴位埋线治疗效果更稳定持久。
Objective:To observe the clinical efficacy of acupoint catgut embedding on migraine.Methods:A total of 60 migraine patients in the outpatient departments of TCM and acupuncture of Shanghai Fifth People′s Hospital from July 2020 to July 2021 were randomly divided into the catgut embedding group and the acupuncture group,with 30 cases in each group.The total number of cases completed was 58,and 2 cases fell off in the acupuncture group,with 30 cases in the catgut embedding group and 28 cases in the acupuncture group.The embedding group was treated with acupoint catgut embedding,while the acupuncture group was treated with ordinary acupuncture.The clinical efficacy,the changes of TCM syndrome score,visual analog score(VAS)score,mean blood flow velocity of cerebrovascular vertebral artery and basilar artery systole were compared between the two groups before and after treatment and one week after treatment.Results:The total scores of TCM syndromes of the catgut embedding group after treatment and one week after treatment were all lower than those of the acupuncture group,and all the differences were statistically significant(P<0.05).The VAS scores of the catgut embedding group after treatment and one week after treatment were all lower than those of acupuncture group,and all the differences were statistically significant(P<0.05).The average blood flow velocities of vertebral artery and basilar artery in the catgut embedding group after treatment and one week after treatment were all lower than those in the acupuncture group,and all the differences were statistically significant(P<0.05).The effective rate was 93.3%in the embedding group and 85.7%in the acupuncture group.There was significant difference between the two groups(P<0.05).However,there was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion:Both acupoint catgut embedding and acupuncture can improve the clinical symptoms of migraine patients,reduce the average blood flow velocity of basilar artery and vert
