摘要
目的 探讨超声内镜联合腹腔镜在消化道黏膜下肿物(SMT)的诊疗中应用价值。方法 收集2018年1月至2020年12月在河北北方学院附属第一医院接受治疗的80例SMT患者,术前均进行超声内镜检查,然后按随机数字表法将其分为研究组(超声内镜辅助腹腔镜切除)和对照组(单纯腹腔镜切除),每组40例。以病理检查结果为金标准,比较超声内镜与病理检查结果的一致性。比较两组术前与术后3 d的胃肠激素指标差异。结果 80例患者的超声内镜与病理诊断结果几乎完全一致(P <0.05)。术后3 d,两组促胃液素(GAS)水平低于治疗前,舒血管肠肽(VIP)水平高于治疗前,且研究组GAS水平高于对照组,VIP水平低于对照组,差异均有统计学意义(P <0.05)。结论 超声内镜诊断效果明确,术中与腹腔镜联合应用使肿瘤层次及周边血管显示明确,可改善胃肠激素分泌情况,临床应用价值显著。
Objective To evaluate the clinical value of ultrasonoscopy combined with laparoscopy in the diagnosis and treatment of gastrointestinal submucosal tumor(SMT). Methods A total of 80 patients with SMT admitted to the First Affiliated Hospital of Hebei North University from January 2018 to December 2020 were enrolled. All patients underwent echoendoscope before surgery and they were divided into study group(echoendoscope-assisted laparoscopy) and control group(laparoscopy alone) according to random number table method, with 40 patients in each group. The consistency between echoendoscope and pathological examination was compared with pathological examination as the gold standard. The differences of gastrointestinal hormone indexes between the two groups were compared before and three days after surgery. Results The results of echoendoscope were almost identical with those of pathological diagnosis in80 patients(P < 0.05). Three days after surgery, the level of gastrin(GAS) was lower than that before treatment, and the level of vasoactive intestinal peptide(VIP) was higher than that before treatment, and the level of GAS in the study group was higher than that in the control group, while the level of VIP was lower than that in the control group, the differences were statistically significant(P < 0.05). Conclusion Echoendoscope has a clear diagnostic effect. Intraoperative combined application with laparoscopy can make tumor levels and peripheral blood vessels clear, improve gastrointestinal hormone secretion, and has significant clinical application value.
