摘要
目的在糖尿病前期和正常糖耐量(normal glucose tolerance,NGT)人群中,探讨午睡与2型糖尿病发病风险之间的关联,并研究该关联是否存在性别和年龄差异。方法本研究的数据来自中国健康与养老追踪调查2011-2018年的基线及随访数据,共纳入8083名45岁及以上的受访者。研究采用依时Cox比例风险模型,对午睡与糖尿病发病风险的关联进行评价,并根据性别和年龄做分层分析。结果在糖尿病前期人群中,>120 min的午睡与糖尿病发病显著相关,风险比(hazard ratio,HR)为1.79,95%的置信区间(confidence interval,CI)为1.16~2.75。分层分析发现关联具有性别和年龄差异,在女性、年龄65岁以下的糖尿病前期人群中,>120 min的午睡与更高的糖尿病风险显著相关,HR及95%CI分别为1.89(1.11~3.22)和2.03(1.23~3.36)。在NGT人群中,午睡与糖尿病发病风险无显著关联,且关联无性别或年龄差异。结论在糖尿病的预防中,糖尿病前期人群,尤其是处于糖尿病前期的女性、年龄小于65岁者,应当避免过长时间的午睡。
Objective To investigate the association between nap and the risk of Type 2 Diabetes Mellitus(T2 DM)in the prediabetes and normal glucose tolerance(NGT)groups respectively,and to investigate whether the association differ by age or gender.Methods The data were obtained from baseline and follow-up data of the China Health and Retirement Longitudinal Survey,CHARLS,incorporating 8083 participants aged 45 years and older.Time-dependent Cox proportional hazards model was used to explore the association between nap and T2 DM.Stratified analyses based on gender and age were conducted.Results In the prediabetes group,a nap of>120 minutes was significantly associated with the increased risk of diabetes with a hazard ratio of 1.79(95%CI:1.16-2.75).Stratified analyses found gender and age differences in the association,with>120 minutes for napping significantly associated with a higher of diabetes in women and the prediabetes population aged under 65 years,with HR and 95%CI of 1.89(1.11-3.22)and 2.03(1.23-3.36),respectively.In the NGT population,napping was not significantly associated with the risk of developing diabetes and there was no gender or age difference in the association.Conclusions Individuals with prediabetes,especially those who are women or under 65 years old,should avoid overlong naps to prevent diabetes.
