糖尿病,遗传易感性和阿尔茨海默病:基于英国生物数据库的回顾性队列研究 被引量:1

摘要 目的研究糖尿病、遗传易感性与阿尔茨海默病风险之间的关联,并进一步探讨潜在的遗传易感性/糖尿病交互作用及对阿尔茨海默病风险的影响。方法基于英国生物数据库,最终有211643例年龄≥60岁的参与者被纳入在本研究中,在基线数据对比后,采用Kaplan-Meier风险曲线和Cox比例风险模型分析不同因素对阿尔茨海默病风险的影响。结果COX比例风险模型表明,年龄越大、社会剥夺指数越高、受教育程度越低、患有心脏病、脑梗死、糖尿病及高血压的参与者有更高的阿尔茨海默病风险。同时,在低AD-GRS(阿尔茨海默病遗传风险评分)和高AD-GRS参与者中,不合并糖尿病能够抵消遗传风险对阿尔茨海默病的风险。结论本研究结果表明随着AD-GRS增加及合并糖尿病,阿尔茨海默病风险越高。同时,在低AD-GRS和高AD-GRS参与者中,不合并糖尿病能够抵消遗传风险对阿尔茨海默病的风险。 Objective To study the association between diabetes and genetic susceptibility and risk of Alzheimer’s disease,and further explore the potential genetic susceptibility/diabetes interaction and their impact on Alzheimer’s disease risk.Methods Based on the UK Biobank,211643 patients aged≥60 were finally included in this study.After comparing the baseline data,Kaplan-Meier risk curve and Cox proportional hazards model were used to analyze the impact of different factors on the risk of Alzheimer’s disease.Results The COX proportional hazards model showed that patients with older age,higher social deprivation index,lower education level,and heart disease,cerebral infarction,diabetes and hypertension had a higher risk of Alzheimer’s disease.At the same time,in patients with low AD-GRS(Alzheimer’s disease genetic risk score)and high AD-GRS,the absence of diabetes could offset the risk of genetic risk for Alzheimer’s disease.Conclusion The risk of Alzheimer’s disease increases with increased AD-GRS and concurrent diabetes.Meanwhile,in patients with low AD-GRS and high AD-GRS,the absence of diabetes can offset the genetic risk for Alzheimer’s disease.
