摘要
目的探讨腰椎间盘突出症的CT及磁共振(MR)诊断价值。方法100例腰椎间盘突出症患者,分别进行CT诊断与MR诊断。比较两种诊断方法的诊断准确率以及不同征象检出情况。结果CT诊断准确率为96.00%(96/100),漏诊误诊率为4.00%(4/100);MR诊断准确率为97.00%(97/100),漏诊误诊率为3.00%(3/100)。MR与CT诊断准确率比较,差异无统计学意义(P>0.05)。CT对椎间盘积气和钙化的检出率分别为54.00%(54/100)、59.00%(59/100),均高于MR诊断的39.00%(39/100)、39.00%(39/100);CT对椎间盘脱出、脊髓变形和硬膜囊压迫的检出率分别为24.00%(24/100)、22.00%(22/100)、30.00%(30/100),均低于MR的39.00%(39/100)、39.00%(39/100)、45.00%(45/100),差异均具有统计学意义(P<0.05)。结论腰椎间盘突出症MR与CT诊断均具有较高诊断价值,但是对不同征象的检出率存在差异。
Objective To discuss the value of CT and magnetic resonance(MR)in the diagnosis of lumbar disc herniation.Methods 100 patients with lumbar disc herniation,all patients were diagnosed by CT and MR respectively.The diagnostic accuracy of the two diagnostic methods and the detection of different signs were compared.Results The diagnostic accuracy of CT was 96.00%(96/100),and the misdiagnosis rate was 4.00%(4/100);the diagnostic accuracy of MR was 97.00%(97/100),and the misdiagnosis rate was 3.00%(3/100).There was no statistically significant difference in the diagnostic accuracy between MR and CT(P>0.05).The detection rates of pneumoconiosis and calcification by CT were 54.00%(54/100)and 59.00%(59/100),which were higher than 39.00%(39/100)and 39.00%(39/100)by MR diagnosis,and the detection rates of intervertebral disc herniation,spinal cord deformation and dural sac compression by CT were 24.00%(24/100),22.00%(22/100),and 30.00%(30/100),which were lower than 39.00%(39/100),39.00%(39/100),and 45.00%(45/100)by MR,and the differences were statistically significant(P<0.05).Conclusion MRI and CT are valuable in the diagnosis of lumbar disc herniation,but the detection rate for different signs is different.
