摘要
目的分析三阴性乳腺癌(TNBC)患者超声征象与腋窝淋巴结转移及预后的关系。方法收集2019年1—12月确诊的TNBC 105例,另选取同期收治的非TNBC 108例。比较2组临床资料,分析TNBC腋窝淋巴结转移患者超声征象,并分析TNBC发生腋窝淋巴结转移的危险因素。结果TNBC组肿瘤直径>5 cm、组织学分级为G2~G3、术后局部复发、腋窝淋巴结转移、远处转移占比均高于非TNBC组,而总生存、无病生存率低于非TNBC组(P<0.05)。TNBC腋窝淋巴结转移组淋巴结纵横比(D/W)≥1.0、肿瘤直径>5 cm、高回声晕、肿瘤内部为不均匀低回声占比高于未转移组(P<0.05)。肿瘤直径>5 cm、肿瘤内部不均匀低回声、有高回声晕、淋巴结D/W≥1.0与TNBC发生腋窝淋巴结转移有相关性(P<0.05)。结论超声征象与TNBC淋巴结转移相关,可为TNBC预后评估提供参考。
Objective To analyze the relationships between ultrasound signs and axillary lymph node metastasis and prognosis in patients with triple-negative breast cancer(TNBC).Methods In total,105 cases of TNBC diagnosed from January 2019 to December 2019 were collected,and 108 cases of non-TNBC treated at the same period were selected.The clinical data of the two groups was compared,the ultrasonic signs of patients with axillary lymph node metastasis of TNBC were analyzed,and the risk factors of axillary lymph node metastasis of TNBC were analyzed.Results The tumor diameter>5 cm,histological grade G2-G3,postoperative local recurrence,axillary lymph node metastasis and distant metastasis in TNBC group were higher than those in non-TNBC group,while the overall survival(OS)and disease-free survival(DFS)rates were lower than those in non-TNBC group(P<0.05).The proportion of lymph node aspect ratio(D/W)≥1.0,tumor diameter>5 cm,hyperechoic halo and uneven hypoechoic inside the tumor in TNBC axillary lymph node metastasis group was higher than that in non-metastasis group(P<0.05).Tumor diameter>5 cm,uneven hypoechoic,hyperechoic halo and lymph node D/W≥1.0 were correlated with axillary lymph node metastasis of TNBC(P<0.05).Conclusion Ultrasound signs are related to lymph node metastasis of TNBC,which can provide reference for the prognosis evaluation of TNBC.
