摘要
目的探究喉罩插管全身麻醉联合骶管阻滞麻醉在小儿中下腹及下肢手术中的安全性和有效性。方法选择2018年4月至2021年4月在鹤山市人民医院择期行小儿中下腹及下肢手术患儿60例为研究对象,依据随机数字表法将其分为对照组(30例,行喉罩插管全身麻醉)和试验组(30例,行喉罩插管全身麻醉联合骶管阻滞麻醉),比较两组患儿围手术期血流动力学指标、术后疼痛程度、苏醒期躁动发生情况及并发症发生情况。结果插入喉罩后即刻(T2)、切皮即刻(T3)、缝合最后一针即刻(T4)、喉罩拔除后即刻(T5)试验组呼吸频率和心率均低于对照组(P<0.05);术后2、6、12 h试验组视觉模拟量表(VAS)评分低于对照组(P<0.05);试验组患儿躁动评分及发生率低于对照组(P<0.05);试验组患儿并发症总发生率稍低于对照组(P>0.05)。结论喉罩插管全身麻醉联合骶管阻滞麻醉可以显著稳定行中下腹及下肢手术患儿围手术期血流动力学指标,降低其术后疼痛程度及躁动发生率,同时安全性较高。
Objective To explore the safety and efficacy of laryngeal mask intubation general anesthesia combined with sacral anesthesia in the middle and lower abdomen and lower limb surgeries in children.Methods A total of 60 children who planned to undergo electivemiddle and lower abdomen and lower limb surgeries in Heshan People’s Hospital from April 2018 to April 2021 were selected as the research subjects.According to the random number table method,they were divided into the control group(30 cases,undergoing laryngeal mask intubation general anesthesia)and the experimental group(30 cases,undergoing laryngeal mask intubation general anesthesia combined with sacral block).The perioperative hemodynamic indexes,postoperative pain,agitation during the revival period and complication occurrence were compared between the two groups.Results The respiratory rate and heart rate of the experimental group were lower than those of the control group immediately after insertion of laryngeal mask(T2),immediately after skin incision(T3),immediately after the last suture(T4)and immediately after laryngeal mask extraction(T5)(P<0.05).The visual analog scale(VAS)score of the experimental group were lower than that of the control group at 2,6,12 h after surgery(P<0.05).The restlessness score and incidence in the experimental group were lower than those in the control group(P<0.05).The total incidence of complications in the experimental group was slightly lower than that in the control group(P>0.05).Conclusion Laryngeal mask intubation general anesthesia combined with sacral block can significantly stabilize the perioperative hemodynamic indexes of children undergoing middle and lower abdomen and lower limb surgeries and reduce postoperative pain and the incidence of agitation,and shows high safety.
