摘要
目的探讨肥胖对支气管哮喘患儿Th1/Th2平衡、肺功能及治疗效果的影响。方法回顾性分析南阳市第一人民医院2019—2020年收治的92例支气管哮喘患儿的临床资料,按体重指数(BMI)分为2组:肥胖组43例,BMI>21 kg·m^(-2);对照组49例,BMI≤21 kg·m^(-2)。2组均给予常规对症治疗3个月。比较2组治疗效果、治疗前后Th1/Th2平衡状态(Th1、Th2和Th1/Th2比值)及肺功能[用力肺活量(FVC)、第1秒用力呼气量(FEV1)、呼气峰值流速(PEF)]。结果与对照组比较,肥胖组治疗总有效率显著降低(P<0.05);与治疗前比较,2组治疗后Th1、Th1/Th2比值、FVC、FEV1、PEF均显著升高,但肥胖组治疗后上述指标升高幅度明显低于对照组(均P<0.05)。结论肥胖可能会引起支气管哮喘患儿Th1/Th2表达失衡,加重肺损害,影响治疗效果。
Objective To investigate the effects of obesity on Th1/Th2 balance,lung function and therapeutic outcome in children with bronchial asthma.Methods Clinical data of 92 children with bronchial asthma treated Nanyang First People’s Hospital from 2019 to 2020 were analyzed retrospectively.According to the body mass index(BMI),these children were divided into obesity group(BMI>21 kg·m^(-2),n=43)and control group(BMI≤21 kg·m^(-2),n=49).Both groups were given routine symptomatic treatment for 3 months.Therapeutic effect,Th1/Th2 balance(Th1 cells,Th2 cells and Th1/Th2 ratio),and pulmonary function(forced vital capacity(FVC),forced expiratory volume in the first second(FEV1)and peak expiratory flow rate(PEF))were compared between the two groups.Results The total effective rate inthe obesity group was lower than that in the control group(P<0.05).After treatment,Th1 cells,Th1/Th2 cell ratio,FVC,FEV1 and PEF significantly increased in both groups,and the increase in the obese group was less than that in the control group(P<0.05).Conclusion Obesity may cause the imbalance of Th1/Th2 cells,aggravate lung damage and affect therapeutic outcome in children with bronchial asthma.
