摘要
近年来脾胃自身分泌功能越来越受到重视,而对其中医归属问题,存在较大的争议。脾主运化的物质基础正是“脾精”,而胰岛素、胃肠激素、肠道菌群、胆汁酸等是现代生理意义上的“脾精”,可归属于脾“脏精”范畴。“助脾散精”,即是通过解脾之困、泄胃之浊及恢复中焦气机以调节脾“脏精”分泌。胆汁酸是重要的肠道代谢产物,可与肠道菌群、肠道激素在肠道发生交互作用。文章从中医“脏精”理论认识胃肠内分泌功能,提出胆汁酸、肠道激素属于脾“脏精”的观点,“胆汁酸-肠道激素”轴调控血糖稳态,即为“脾精”气化代谢影响血糖的稳定,从而丰富“助脾散精”内涵,为中医治疗糖尿病提供科研及临床思路。
In recent years,more and more attention has been paid to the secretory function of the spleen and stomach,and there is a great controversy over its ownership in traditional Chinese medicine.The material basis for the spleen to govern the transport and transformation is the‘spleen essence’.Insulin,gastrointestinal hormones,intestinal flora,bile acids,etc.are the‘spleen essence’in the modern physiological sense,which can belong to the category of spleen‘visceral essence’.‘Invigorating spleen for dispersing essence’is to regulate the secretion of spleen‘visceral essence’by relieving the obstruction of the spleen,purging the turbid stomach and restoring the movement of qi in the middle-jiao.Bile acid,as an important intestinal metabolite,interacts with intestinal flora and intestinal hormones.Based on the theory of the‘visceral essence’of traditional Chinese medicine to understanding the endocrine functions of gastrointestinal,we point out that bile acid and intestinal hormones belong to spleen‘visceral essence’.The axis of‘bile acid-intestinal hormone’regulates blood glucose homeostasis,that is,qi metabolism of spleen viscera‘essence’affects the stability of blood glucose.Therefore,it enrichs the content of‘invigorating spleen for dispersing essence’,and provides scientific research and clinical ideas for the treatment of diabetes with traditional Chinese medicine.
