摘要
目的分析新型冠状病毒肺炎疫情发生前后社区卫生服务中心基本公共卫生服务项目成本和工作量的变化。方法收集广西南宁某社区卫生服务中心疫情发生前后(2019年和2020年)的业务活动收支和工作量数据(包括服务量和人口数据)。运用当量法进行成本测算,分析疫情发生前后12项13大类基本公共卫生服务项目人均成本和工作量变化的特点。结果疫情发生后,该机构基本公共卫生服务项目人均成本上涨22.61元。疫情发生后,基本公共卫生服务项目明细的当量值结构有所变动:从横向看,项目当量值增幅最大的是慢性病患者健康管理(高血压)和中医药健康管理,当量值占比增长率分别为22.93%和19.85%;从纵向看,建立居民健康档案的当量值仍居首位,其次是老年人健康管理。结论新型冠状病毒肺炎疫情发生后,该机构项目人均成本有所上涨,重点人群对于预防和控制慢性病的需求增加,对国家基本公共卫生服务项目的认可度提升。坚持预防为主、中西医并重,持续提升社区卫生服务中心应对疫情能力,对打赢疫情防控战役起到重要作用。
Objective To analyze the changes in the costs and workloads of basic public health service projects in a community health service center before and after the outbreak of coronavirus disease 2019(COIVD-19)pandemic.Methods The revenues and expenditures of business activities and the data of workloads(including service volume and demographic data)before and after the pandemic outbreak(2019 and 2020)were collected from a community health service center in Nanning City,Guangxi.Costs′measurement was performed using the equivalent method,and the characteristics of changes in per capita cost and workload of basic public health service projects(12 sectors and 13 major categories)before and after the pandemic outbreak were analyzed.Results After the pandemic outbreak,the per capita cost of basic public health service projects in this institution increased by 22.61 yuans.After the pandemic outbreak,the equivalent structure of basic public health service projects breakdown had changed:transversely,the maximum increment in the project equivalent value was found in health management for patients with chronic diseases(hypertension)and traditional Chinese medicine health management,and the growth rates of their equivalent values shares were 22.93%and 19.85%,respectively;longitudinally,the equivalent value of the establishment of residents′health records still ranked the first,followed by that of health management for elderly people.Conclusion After the outbreak of COVID-19 pandemic,the per capita cost of the projects in this institution increased,major populations′demands for the prevention and control of chronic diseases were on the rise,and the approval for national basic public health service projects was elevated.Adhering to prevention as the main focus,paying equal attention to both traditional Chinese medicine and Western medicine,and improving the capability of community health service centers to deal with the pandemic constantly play an important role in winning the battle for pandemic prevention and control.
