摘要
目的探讨经阴道超声联合经腹超声在子宫肌瘤患者中的诊断价值及效能ROC曲线。方法选择2019年4月至2021年3月在南昌大学第四附属医院就诊临床疑诊为黏膜下子宫肌瘤的有性生活女性患者40例,根据检查顺序随机分为对照组和观察组,每组各20例。所有患者均具有完整的经病理检查结果(金标准),对照组采用经腹超声检查,观察组联合采用经阴道超声检查,比较两组诊断价值,并分析其超声特点;绘制ROC曲线,分析经阴道超声联合经腹超声在子宫肌瘤患者中的诊断效能。结果40例疑似子宫肌瘤患者病理检查均最终确诊。观察组联合检查确诊19例,确诊率为95.00%;对照组经腹超声检查确诊16例,确诊率为80.00%,观察组确诊率低于对照组(P<0.05);观察组检出率高于对照组;ROC曲线结果表明:经阴道超声联合经腹部超声在子宫肌瘤患者中的诊断敏感度和特异度高于单一经阴道超声、经腹部超声(P<0.05)。结论经阴道超声联合经腹超声应用子宫肌瘤患者中诊断符合率较高,两种超声联合可获得较高诊断敏感度和特异度,具有典型的超声影像特点,能为临床诊疗提供参考依据。
Objective To investigate the diagnostic value and efficacy of transvaginal ultrasound combined with transabdominal ultrasound in patients with uterine fibroids by receiver operating characteristic(ROC)curve.Methods A total of 40 female patients with sexual intercourse who were clinically suspected of submucosal uterine fibroids attending our hospital from April 2019 to March 2021 were selected as study subjects.They were randomly divided into the control group and the observation group according to the time order of receiving examination,with 20 cases in each group.All patients had complete pathological findings(gold standard).The control group underwent trans-abdominal ultrasound,while the observation group underwent transvaginal ultrasound combine with transabdominal ultrasound.The diagnostic value was compared between the two groups,and the ultrasound characteristics were analyzed.ROC curve was drawn to analyze the diagnostic efficacy of transvaginal ultrasound combined with transabdominal ultrasound in patients with uterine fibroids.Results All the 40 patients with suspected uterine fibroids were finally accurately diagnosed by pathological examination.19 patients in the observation group were accurately diagnosed by combined examination,with an accurate diagnosis rate of 95.00%,while 16 patients in the control group were accurately diagnosed by transabdominal ultrasound,with an accurate diagnosis rate of 80.00%,the diagnosis rate of observation group was higher than the control group(P<0.05).The detection rate in the observation group was higher than that in the control group.The ROC curve showed that the diagnostic sensitivity and specificity of transvaginal ultrasound combined with transabdominal ultrasound were higher than those of transvaginal ultrasound and transabdominal ultrasound alone in patients with uterine fibroids(P<0.05).Conclusion The combination of transvaginal ultrasound and transabdominal ultrasound has a higher diagnostic compliance rate in patients with uterine fibroids,with higher diagno
