摘要
目的:观察外用活血通络方治疗糖尿病周围神经病变患者的临床疗效。方法:选取80例气虚血瘀型糖尿病周围神经病变患者,随机平均分为对照组和治疗组。对照组给予常规综合治疗,治疗组在对照组治疗基础上给予外用活血通络方,两组疗程均为2周。比较两组患者临床疗效、中医症状积分、血糖水平、密歇根糖尿病神经病变评分(Michigan diabetic neuropathy score,MDNS)及神经传导速度等。结果:治疗组有效率为85.00%,高于对照组的65.00%,差异有统计学意义(P<0.05)。治疗后,两组患者中医症状积分均低于治疗前,差异有统计学意义(P<0.05);治疗组治疗后中医症状积分低于同期对照组(P<0.05)。治疗后,两组患者空腹血糖、餐后2小时血糖、糖化白蛋白水平均低于治疗前(P<0.05)。治疗后,两组患者大脚趾感觉、四肢远端肌力、腱反射及MDNS均低于本组治疗前,差异均有统计学意义(P<0.05)。治疗后,两组患者神经传导速度均高于治疗前,且治疗组高于同期对照组,差异均有统计学意义(P<0.05)。结论:外用活血通络方治疗糖尿病周围神经病变疗效显著,可显著降低中医症状积分、MDNS、血糖水平,提高神经传导速度。
Objective:To observe the clinical effect of external application of Huoxue Tongluo Recipe in the treatment of patients with diabetic peripheral neuropathy.Methods:Select 80 patients with diabetic peripheral neuropathy with syndrome differentiation of Qi deficiency and blood stasis type,and randomly divide them into a control group and a treatment group,with 40 patients in both groups.The control group was given conventional comprehensive treatment,and the treatment group was given external Huoxue Tongluo Recipe on the basis of the treatment of the control group.The course of treatment in both groups was 2 weeks.The clinical efficacy,TCM symptom score,blood glucose level,Michigan diabetic neuropathy score(MDNS)and nerve conduction velocity were compared between the two groups.Results:The effective rate of the treatment group was 85.00%,which was higher than that of the control group,65.00%,and the difference was statistically significant(P<0.05).After treatment,the scores of TCM symptoms in the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycated albumin in the two groups were lower than those before treatment(P<0.05).After treatment,the sensation of big toe,muscle strength of distal limbs,tendon reflex and MDNS in the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After treatment,the nerve conduction velocity in the two groups was higher than before treatment,and the treatment group was higher than the control group at the same period,the differences were statistically significant(P<0.05).Conclusion:External application of Huoxue Tongluo Recipe has a significant curative effect in the treatment of diabetic peripheral neuropathy,which can significantly reduce TCM symptom scores,MDNS and blood sugar levels,and improve nerve conduction velocity.
