摘要
目的分析德江及周边地区结石患者的结石成分差异,探讨该地区泌尿系结石的临床防治方法,为预防其疾病的复发提供参考依据。方法收集2018年1月至2020年9月本院收治的298例结石患者的结石样本并分析结石成分。结果298例结石患者共检出6种结石成分,单一成分结石检出率为1.01%,混合成分的结石检出率为98.99%,其中两种成分的混合结石为92.95%(277/298),一水草酸钙+二水草酸钙为主的患者为62.75%(187/298),三种成分的混合结石的患者为6.04%(18/298)。各种不同成分结石在不同部位分布之间比较,差异有统计学意义(P<0.001);其中一水草酸钙+二水草酸钙结石在输尿管中最多,且该类结石在不同部位的分布比较,差异有统计学意义(χ^(2)=9.189,P=0.027);胆固醇、胆固醇+胆红素结石只存在胆囊中且所占比例较小,分布差异均有统计学意义(均P<0.001)。男性和女性患者在不同成分结石发生率中分布比较,差异均无统计学意义(均P>0.05)。年龄31~35岁患者为结石高发人群,各部位结石在不同年龄段的分布比较,差异有统计学意义(χ^(2)=32.966,P<0.001);其中膀胱结石在31~35岁患者中比例最大,其在不同年龄段的分布比较,差异有统计学意义(χ^(2)=30.215,P<0.001)。在不同年龄段的其他部位结石分布比较,差异均无统计学意义(均P>0.05)。各种不同成分的结石在不同的年龄段分布比较,差异无统计学意义(χ^(2)=16.414,P=0.746)。结论德江及周边地区结石患者中,男性的发病率较高,中年人高发,其中混合结石占绝大多数,结石成分以一水草酸钙、二水草酸钙和碳酸磷灰石为主,多发于输尿管。因此,根据泌尿系结石发生的病因制定其防治方案具有重要意义。
Objective To analyse the differences in the composition of stones in patients with stones in Dejiang and surrounding areas,and explore the clinical prevention and treatment methods of urinary stones in this area,and provide reference for preventing the recurrence of their diseases.Methods The stone samples of 298 patients with calculi admitted to our hospital from January 2018 to September 2020 were collected and the stone composition was analyzed.The clinical characteristics of the patients were analyzed and compared combined with clinical data.Results A total of 6 stone components were detected in 298 patients with calculi,single component calculi accounted for only 1.01%,and mixed component calculi accounted for 98.99%.The combined calculus of the two components was 92.95%(277/298).There were statistically significant differences in the distribution of calculi with different components in different parts(P<0.001),and calcium oxalate monohydrate+calcium oxalate dihydrate calculi were the most common in ureter,and the difference in the distribution of such calculi in different parts was statistically significant(χ^(2)=9.189,P=0.027).Cholesterol and cholesterol+bilirubin stones only existed in gallbladder and accounted for a small proportion,with statistically significant differences in distribution(all P<0.001).Patients aged from 31 to 35 years were the population with high incidence of calculi,and there was statistical significance in the distribution of calculi in different parts at different ages(χ^(2)=32.966,P<0.001).Among them,the proportion of bladder calculi was the largest in patients aged from 31 to 35 years,and the distribution of bladder calculi in different age groups was statistically significant(χ^(2)=30.215,P<0.001).There was no significant difference in the distribution of stones with different components in different age groups(χ^(2)=16.414,P=0.746).Conclusions Stone patients in Dejiang and surrounding areas,the incidence of men is relatively high,and the incidence of middle-aged and elderly
