摘要
目的分析彩色多普勒超声在糖尿病肾病临床诊断中的价值。方法分析对象为2019年1月—2021年1月在该院就诊的100例糖尿病肾病患者,纳入研究组;选择同期在该院实施健康体检的健康者100名,纳入对照组。均采用彩色多普勒超声对两组实施诊断,对比和分析超声诊断结果。结果研究组患者肾叶间动脉(IRA)、肾窦部段动脉(SRA)、肾门处主肾动脉(MRA)的舒张末期血流速度(EDV)、收缩期峰值血流速度(PSV)、血流阻力指数(RI)与对照组比较,差异有统计学意义(P<0.05);对比对照组,研究组肾脏厚度较厚,肾脏宽度较宽,肾脏长度较长,差异有统计学意义(P<0.05);对比对照组,研究组患者颈部总动脉内径值明显较高,颈动脉内膜中层厚度明显较高,差异有统计学意义(P<0.05);研究组患者出斑率明显比对照组高,差异有统计学意义(P<0.05)。结论彩色多普勒超声在糖尿病肾病临床诊断中具有较高的价值,糖尿病肾病患者在血流动力学、肾脏形态、颈动脉结构方面均呈现异常情况。
Objective To analyze the value of color Doppler ultrasonography in the clinical diagnosis of diabetic nephropathy.Methods A total of 100 patients with diabetic nephropathy who were treated in the hospital from January 2019 to January 2021 were included in the study group.And 100 healthy people who underwent physical examination in the hospital at same period were selected and included in the control group.Both groups were diagnosed by color Doppler ultrasound,and the results of ultrasound diagnosis were compared and analyzed.Results The blood flow velocity of the renal interlobar artery(IRA),segmental renal sinus artery(SRA),and main renal artery(MRA)at the renal hilum,end-diastolic blood flow velocity(EDV),peak systolic blood flow velocity(PSV),and the flow resistance index(RI)in the study group were compared with that of the control group,and the difference was statistically significant(P<0.05).Compared with the control group,the kidney thickness was thicker,the width of the kidney was wider,and the length of the kidney was longer of the study group,and the difference was statistically significant(P<0.05);compared with the control group,the internal diameter of the common cervical artery in the study group was significantly higher,and the carotid intima-media thickness was significantly higher,and the difference was statistically significant(P<0.05).The incidence of plaque in the study group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Color Doppler ultrasonography has a high value in the clinical diagnosis of diabetic nephropathy.It shows abnormality in hemodynamics,kidney morphology and carotid artery structure in diabetic nephropathy patients.
