摘要
糖尿病肾病(DN)是一种常见且难治的糖尿病微血管并发症。咖啡酸(又名3,4-二羟基肉桂酸)是一种广泛分布于自然界的酚类化合物,已被证明对DN治疗有效。咖啡酸独特而简单的化学结构使其具有良好的抗氧化、抗炎、抗病毒等药理活性,被认为是治疗糖尿病肾病的良好替代药物。咖啡酸能有效通过降低尿蛋白、保护肾功能治疗DN,其药理机制与降糖、自噬、抗炎、抗氧化有关,主要通过平衡血糖水平、调节AMPK、PI3K/AKT、JNK-Bcl-2等途径实现。本文就咖啡酸治疗糖尿病肾病的保护作用和药理机制做一综述,为临床上糖尿病肾病的预防及治疗提供新的方向和思路。
Diabetic nephropathy(DN)is a common and intractable diabetic microvascular complication.Caffeic acid(also known as 3,4-dihydroxycinnamic acid),a phenolic compound widely distributed in nature,has been proven to be effective for DN treatment.Caffeic acid is considered as the good alternative medicines for DN because of its unique and simple chemical structure,also with good pharmacological activities such as anti-oxidation,anti-inflammation and anti-virus.Caffeic acid can effectively treat DN by reducing urinary protein and protecting renal function.Its pharmacological mechanism involves anti-diabetic,autophagy,anti-inflammation and anti-oxidation,which is achieved by balancing the blood sugar level and regulating the following pathways:AMPK,PI3K/AKT,JNK-Bcl-2,and so on.This paper reviews the protective effect and pharmacological mechanism of caffeic acid in the treatment of diabetic nephropathy,in order to provide new directions and ideas for the prevention and treatment of diabetic nephropathy in clinic.
