帕金森病的PET分子影像学表现

摘要 帕金森病(Parkinson’s Disease,PD)是老年人第二大常见的神经退行性疾病,目前虽然尚未确定PD的高度特异性和敏感性的成像生物标志物,但正电子发射断层成像术(Positron Emission Tomography,PET)提高了大家对这种疾病的病理生理学和神经病理学的认识,并且经常被用作这种疾病鉴别诊断的支持标准。各种PET示踪剂可用于评估PD患者的运动和认知功能障碍。^(18)F-氟代脱氧葡萄糖(^(18)F-FDG)PET有助于评估患有认知功能障碍和痴呆的PD患者的皮质代谢。针对突触前多巴胺功能的PET示踪剂和突触后多巴胺受体用于评估PD的运动功能障碍。近年来,新的成像放射性示踪剂,如针对单胺能神经元和胆碱能神经元的PET示踪剂,能够评估可能参与PD非运动症状(包括抑郁症、疲劳、睡眠障碍和认知障碍)病理生理学的非多巴胺能脑通路。本文就PD中使用的PET分子影像学表现进行概述。 Parkinson’s disease(PD)is the second most common progressive neurodegenerative disorder.While highly specific and sensitive imaging biomarkers of individual parkinsonism have not been identified yet,molecular Positron Emission Tomography(PET)imaging have improved our knowledge of the physiopathology and neuropathology of this disorder and are often used as supportive criteria for the differential diagnosis of this condition.Various PET tracers are available to assess motor and cognitive dysfunctions in PD.^(18)F-Fludeoxyglucose(^(18)F-FDG)PET help to evaluate cortical metabolism in PD patients with cognitive dysfunction and dementia.PET tracers targeting presynaptic dopaminergic function and postsynaptic dopamine receptors are used to assess motor dysfunction in PD.In recent years,the growing availability of new imaging radiotracers for monoaminergic and cholinergic neurons has enabled the evaluation of the non-dopaminergic brain pathways that are likely to be involved in the pathophysiology of non motor symptoms of PD,including depression,fatigue,sleep disorders,and cognitive impairment.This chapter will provide a state-of-the-art overview of the use of PET in parkinsonisms.
