摘要
目的分析狂犬病人免疫球蛋白在狂犬病Ⅲ级暴露中使用情况调查情况。方法选择医院2019年7月-2020年1月收诊的狂犬病Ⅲ级暴露患者118例,通过病案信息收集和分析Ⅲ级暴露患者的年龄、发病月份、被咬伤部位、潜伏时间与受伤程度和狂犬病人免疫球蛋白注射情况。结果118例患者中注射狂犬病人免疫球蛋白有70例,注射率为59.32%。<15岁的Ⅲ级暴露患者注射狂犬病人免疫球蛋白率最高,其次是15~25岁、25~30岁、40~50岁,而50~60岁最低。季注射率由高到低依次是3季度22例,注射率为95.65%;2季度30例,注射率为88.24%;1季度,28例,注射率为53.85%;4季度10例,其中注射率50.00%;注射率由高到低依次头面部6例,注射率100%;、躯干30例,注射率70.00%;四肢71例,注射率是60.56%。注射率由高到低依次是15天以内6例,注射率83.33%;1个月以内20例,注射率85%;1~3个月21例,注射率为71.43%;3~6个月以上71例,注射率为46.48%。结论狂犬病人免疫球蛋白在狂犬病Ⅲ级暴露中使用率较低,其中不同年龄、季节、咬伤部位和潜伏期的注射情况均有差别,要提高狂犬病人免疫球蛋白注射率,做好相关防护。
Objective To analyze the use rate of rabies immunoglobulin in patients exposed to gradeⅢrabies.Methods A total of 118 cases of rabiesⅢexposure patients in our hospital from July 2019 to January 2020 were selected.Data including age,month of onset,location of the bite,incubation time and degree of injury were collected through medical records.Then the immunoglobulin injection status was also analyzed.Results Among the 118 patients,70 cases were injected with rabies immunoglobulin,with a rate of 59.32%.Among different age groups,the rabies immunoglobulin injection rate was the highest among patients under 15~25 years old group,followed by 25~30 years old group,40~50 years old group,and was the lowest in 50~60 years old group.The quarterly injection rate was the highest in the third quarter(22 cases,95.65%),followed by the second quarter(30 cases,88.24%),the first quarter(28 cases,53.85%),and was the lowest in the fourth quarter(10 cases,50.00%);Among different bite sites,the site with the highest injection rate was head and face(6 cases,100%),followed by trunk(30 cases,70.00%)and was the lowest in limbs(71 cases,60.56%).The injection rates among different incubation periods were the highest within 15 days(6 cases,83.33%),followed by within 1 month(20 cases,85%),1 to 3 months(21 cases,71.43%),and were the lowest in 3 to 6 months(71 cases,46.48%).Conclusion The use rate of rabies immunoglobulin in patients exposed to grade III rabies is relatively low,and the injection rate varies with the ages,seasons,bite sites and incubation periods,while it is of vital importance to increase the immunoglobulin injection rate of rabies patients.
