神经生长因子联合抗氧化治疗肢骨折内固定术后骨折愈合、骨代谢及氧化应激反应程度的影响 被引量:10

Effect of NGF combined with antioxidant on fracture healing,bone metabolism and oxidative stress after fracture fixation in limbs fracture

摘要 目的 研究神经生长因子(NGF)联合抗氧化治疗对四肢骨折内固定术后骨折愈合、骨代谢及氧化应激反应程度的影响。方法 前瞻性选取2019年6月至2021年3月期间因四肢骨折在武汉科技大学附属普仁医院进行切开复位内固定手术的78例患者作为研究对象,采用随机数字表法将其分为两组,每组39例。对照组术后采用常规治疗,实验组术后采用NGF联合抗氧辅助治疗。随访骨折愈合情况;测定手术前后血清中骨代谢指标[骨特异性碱性磷酸酶(BALP)、骨保护素(OPG)、Ⅰ型原胶原氨基端延长肽(PINP)、核因子KB受体活化因子配体(RANKL)、抗酒石酸酸性磷酸酶(TRACP-5b)]及氧化应激指标[丙二醛、晚期氧化蛋白产物(AOPP)、超氧化物歧化酶(SOD)、血红素氧合酶-1(HO-1)]的含量。结果 实验组患者骨痂形成时间、骨折愈合时间为(6.58±0.87)、(13.85±1.68)周,显著短于对照组[(9.94±1.13)、(18.13±2.03)周],差异均有统计学意义(P <0.05)。术后1、3个月,两组患者骨折区的骨密度显著升高,且实验组患者骨折区的骨密度显著高于对照组,差异均有统计学意义(P <0.05)。术后1周时,两组患者血清中BALP、OPG、PINP的含量均较术前显著升高,RANKL、TRACP5b的含量均较术前显著降低,且实验组BALP、OPG、PINP的含量为(26.52±3.32) U/m L、(0.33±0.05) ng/m L、(43.94±5.84) ng/m L,显著高于对照组[(22.36±3.12)U/m L、(0.25±0.04) ng/m L、(34.21±4.48) ng/m L],RANKL、TRACP5b的含量为(64.58±7.85) ng/m L、(3.26±0.52)μg/m L,显著低于对照组[(75.23±8.69) ng/m L、(4.41±0.57)μg/m L],差异均有统计学意义(P <0.05)。术后1周,两组患者血清中丙二醛、AOPP的含量均较术前显著降低,SOD、HO-1的含量均较术前显著升高,且实验组患者丙二醛、AOPP的含量为(6.31±0.78)、(14.52±1.76)μmol/L,显著低于对照组[(8.84±1.03)、(20.16±2.84)μmol/L],SOD、HO-1的含量为(98.31±10.32)、(61.36±7.68) U/m L,显著高于对� Objective To study the effect of nerve growth factor(NGF)combined with antioxidant on fracture healing,bone metabolism and oxidative stress after fracture fixation in limbs fracture.Methods A total of 78 patients who underwent open reduction and internal fixation due to limb fractures in the Puren Hospital Affiliated to Wuhan University of Science and Technology from June 2019 to March 2021 were prospectively selected as the research subjects.They were divided into two groups according to the random number table,with 39 cases in each group.The control group received postoperatively conventional therapy,the experimental group received postoperatively NGF combined with antioxidant therapy.After operation,the healing of fracture was followed up.The bone metabolism index[bone specific alkaline phosphatase(BALP),osteoprotegerin(OPG),propeptide of type I procollagen(PINP),nuclear factor KB receptor activator ligand(RANKL),tartrate-resistant acid phosphatase(TRACP-5b)]and oxidative stress index[malondialdehyde(MAD),advanced oxidation protein product(AOPP),superoxide dismutase(SOD),heme oxygenase-1(HO-1)]in serum were measured before and after operation.Results The callus formation time and fracture healing time in the experimental group were(6.58±0.87)and(13.85±1.68)weeks,which were significantly shorter than those in the control group[(9.94±1.13)and(18.13±2.03)weeks],and the differences were statistically significant(P<0.05).At 1 and 3 months after operation,the bone mineral density in the fracture area of the two groups was significantly increased,and the bone mineral density in the fracture area of the experimental group was significantly higher than that of the control group,the differences were statistically significant(P<0.05).At 1 week after the operation,the levels of serum BALP,OPG and PINP in the two groups were significantly increased compared with those before the operation,and the levels of serum RANKL and TRACP5b were significantly decreased compared with those before the operation,and the levels of se
