摘要
目的为老年患者合理使用中药注射剂提供参考。方法收集医院2016年至2021年上报的老年(≥60岁)患者使用中药注射剂发生药品不良反应(ADR)的报告,并依据数据绘制帕累托图,分析发生ADR老年患者的一般情况,老年患者使用后发生ADR的中药注射剂品种,ADR累及器官/系统,用药后发生ADR时间,临床科室及合并用药情况对ADR发生的影响。结果共收集134例ADR,其中男50例,女84例;药师上报108例,医师上报24例,护士上报2例。帕累托图分析结果显示,使用后发生ADR的主要中药注射剂为舒血宁注射液和丹红注射液,未发生严重ADR,累及器官/系统以皮肤及其附件、血液及造血系统、中枢及外周神经系统、全身性及循环系统为主,ADR多在用药1~7天内发生(70例,52.24%),急诊内科上报的ADR最多(36例,26.87%),多重(>5种)用药的有84例(91.30%)。结论老年患者使用中药注射剂应全面考虑年龄、基础疾病与合并用药,重点关注皮疹、出血、心血管系统症状,及时采取干预措施,以减少ADR的发生。
Objective To provide a reference for the rational use of traditional Chinese medicine(TCM)injection in elderly patients.Methods The reports of adverse drug reactions(ADRs)related to TCM injection in elderly patients(≥60 years old)reported by the hospital from 2016 to 2021 were collected,and the Pareto chart was drawn according to the data to analyze the general situation of elderly patients with ADR,the varieties of TCM injections with ADRs in elderly patients,the systems/organs involved in ADRs,onset of ADR after medication,the influence of clinical departments and combined medication on the occurrence of ADR.Results A total of 134 ADR reports were collected,including 50 males and 84 females,among which 108 cases were reported by pharmacists,24 cases were reported by doctors and two cases were reported by nurses.Pareto chart showed that ADRs were mainly related to Shuxuening Injection and Danhong Injection,and no severe ADRs occurred.The organs/systems involved in ADRs were mainly skin and its accessories,blood and hematopoietic system,central and peripheral nervous system,systemic damage and circulatory system.ADRs mostly occurred within 1-7 d of medication(70 cases,52.24%),the most ADRs(36 cases,26.87%)were reported by the emergency department,and 84 cases(91.30%)were treated with multiple drugs(more than five kinds).Conclusion Administration of TCM injections in elderly patients should fully consider the patients′age,underlying diseases and concurrent medications,focus on rash,bleeding and cardiovascular symptoms,and take timely intervention measures to reduce the occurrence of ADRs.
