miR-146b-3p在妊娠期糖尿病患者母血和脐血中的表达水平及对胰岛素抵抗的影响 被引量:2

摘要 目的探讨miR-146b-3p在妊娠期糖尿病(gestational diabetes mellitus,GDM)患者母血和脐血中的表达水平及其与胰岛素抵抗(insulin resistance,IR)的关系。方法将中国人民解放军北部战区总医院收治的120例GDM患者作为GDM组,另外选取116例健康体检孕妇作为对照组。记录两组孕妇及新生儿一般资料,分别检测空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)、总胆固醇(total cholesterol,TG)、甘油三酯(triacylglycerol,TC)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)和糖化血红蛋白(glycosylated hemoglobin,HbAlc)水平,计算胰岛素抵抗指数(homeostasis model assessment-insulin resistance,HOMA-IR),比较两组孕妇母血和脐血中miR-146b-3p的表达水平,并分析其与IR的相关性。结果相对于对照组,GDM组母血FBG、FINS、HOMA-IR、TC、TG、HbAlc、LDL-C和脐血胰岛素显著升高,HDL-C和脐血血糖水平显著降低(均P<0.05)。miR-146b-3p在GDM组母血和脐血中的表达高于对照组(均P<0.05)。母血miR-146b-3p和脐血miR-146b-3p在GDM中的曲线下面积(area under curve,AUC)分别为0.8243和0.8038,联合AUC为0.9027(均P<0.05)。miR-146b-3p在母血或脐血中的水平分别与母血或脐血HOMA-IR显著正相关(均P<0.05)。结论miR-146b-3p在GDM患者母血和脐血中的表达具有诊断价值,与IR具有显著相关性。 Objective To investigate the expression level of miR-146b-3p in maternal blood and cord blood of patients with gestational diabetes mellitus(GDM)and its effect on insulin resistance(IR).Method GDM patients admitted to General Hospital of the Northern Theater of the Chinese People's Liberation Army were selected as the research objects(n=120),and healthy pregnant women were selected as the control group(n=116).The clinical data of pregnant women and newborns in the two groups were recorded.Fasting blood glucose(FBG),fasting insulin(FINS),total cholesterol(TG),triacylglycerol(TC),and low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C)and glycosylated hemoglobin(HbAlc)levels were tested.The homeostasis model assessment-insulin resistance(HOMA-IR)was calculated.We compared the expression levels of miR-146b-3p in the maternal blood and cord blood of pregnant women in the two groups,and analyzed its correlation with IR.Result Compared with the control group,maternal blood FBG,FINS,HOMA-IR,TC,TG,HbAlc,LDL-C and cord blood insulin were significantly higher and HDL-C and cord blood glucose levels were significantly lower in the GDM group(all P<0.05).The expression level of miR-146b-3p in maternal blood and cord blood was higher in the GDM group than in the control group(all P<0.05).The areas under curve(AUC)of maternal blood miR-146b-3p and cord blood miR-146b-3p in GDM were 0.8243 and 0.8038,respectively,and the combined AUC was 0.9027(all P<0.05).The levels of miR-146b-3p in maternal or cord blood were significantly and positively correlated with maternal or cord blood HOMA-IR,respectively(both P<0.05).Conclusion The expression of miR-146b-3p in maternal blood and cord blood of GDM patients has diagnostic value and correlates significantly with IR.
