摘要
目的观察艾灸联合通阳泄浊自拟方治疗糖尿病下肢血管病变的临床疗效。方法选取2018年10月—2020年5月上海市嘉定区中医医院内分泌科收治的糖尿病下肢血管病变患者126例,随机分为对照组、中药组、联合组。3组均给予常规治疗,在此基础上对照组予以贝前列素钠片口服,中药组给予通阳泄浊自拟方口服,联合组给予艾灸联合通阳泄浊自拟方口服,3组均治疗12周。记录治疗前后各组血糖[空腹血糖(FPG)、餐后2 h血糖(2hPG)]、果糖胺(FMN)水平、疼痛视觉模拟评分(VAS)、踝肱指数(ABI)、血管内径、无痛行走最大距离,并统计各组临床疗效。结果治疗后3组FPG、2hPG、FMN水平均较治疗前明显降低(P均<0.05),中药组及联合组均明显低于对照组(P均<0.05),联合组均明显低于中药组(P均<0.05)。治疗后3组VAS评分、超声积分、ABI均较治疗前下降(P均<0.05),下肢动脉内径均较治疗前增宽(P均<0.05),无痛行走最大距离较均较治疗前增加(P均<0.05),且中药组及联合组各指标改善情况均优于对照组(P均<0.05),联合组各指标改善情况均优于中药组(P均<0.05)。对照组、中药组及联合组临床总有效率分别为62.16%(23/37)、69.44%(25/36)、77.14%(27/35),中药组和联合组总有效率高于对照组(P均<0.05)。结论艾灸联合通阳泄浊自拟方治疗有利于控制糖尿病下肢血管病变患者血糖,减轻疼痛,扩张下肢血管内径,降低ABI,增加无痛行走最大距离。
Objective It is to observe the clinical effect of moxibustion combined with Tongyang Xiezhuo self-made decoction in the treatment of diabetic lower extremity vascular disease.Methods A total of 126 patients with diabetic lower extremity vascular disease who were admitted to the Endocrinology Department of Shanghai Jiading District Hospital of Traditional Chinese Medicine from October 2018 to May 2020 were selected and randomly divided into control group,traditional Chinese medicine group and combination group.The three groups were all given routine treatment,additionally,the control group was orally given beprost sodium tablets,the traditional Chinese medicine group was orally given Tongyang Xiezhuo self-made decoction,and the combination group was treated with moxibustion combined with Tongyang Xiezhuo self-made decoction.Both groups were treated for 12 weeks.The levels of blood glucose[fasting blood glucose(FPG),2 h postprandial blood glucose(2hPG)],fructosamine(FMN),scores of pain visual analog scale(VAS),ankle-brachial index(ABI),vascular diameter,no painful walking maximum distance before and after treatment were recorded,and the clinical efficacy was evaluated in the three groups.Results After treatment,the levels of FPG,2hPG and FMN in the three groups were significantly lower than those before treatment(all P<0.05),and the indexes in the traditional Chinese medicine group and the combination group were significantly lower than those in the control group(all P<0.05),the combination group was significantly lower than the traditional Chinese medicine group(P<0.05).After treatment,the VAS scores,ultrasound scores and ABI in the three groups were significantly decreased than those before treatment(all P<0.05),the inner diameter of lower extremity arteries were all wider than those before treatment(all P<0.05),and the maximum distance of painless walking was increased than those before treatment(all P<0.05),the improvement of each index in the traditional Chinese medicine group and the combination group was bet
