摘要
目的:探讨浮针疗法对痉挛型脑瘫下肢矫形术后疼痛的临床治疗效果。方法:选择42例痉挛型脑瘫患者,均施行下肢矫形手术,石膏固定4~6周,按就诊的奇偶顺序分为治疗组和对照组,每组21例。治疗组为浮针加常规治疗,对照组为常规治疗。采用视觉模拟评分法(VAS)记录拆石膏后第1天、第5天、第10天的踝关节疼痛程度,剪切波弹性成像检查拆石膏后第1天、第10天的腓肠肌E值,运用统计学对数据进行分析。结果:两组病例基线期资料无统计学差异,P>0.05。两组病例拆石膏后第1天、第5天、第10天的VAS组内比较和组间比较均有统计学意义,P<0.05。两组病例拆石膏后第1天、第10天的腓肠肌E值组内比较和组间比较均有统计学意义,P<0.05。结论:浮针疗法对缓解痉挛性脑瘫下肢矫形术后的关节疼痛有较好的止痛效果,并通过剪切波弹性成像检查,发现浮针治疗可更有效降低腓肠肌硬度,缓解痉挛,对痉挛型脑瘫的术后康复有良好的临床疗效。
Objective:To investigate the clinical efficacy of floating acupuncture therapy for postoperative pain following orthopedic surgery of lower extremity in patients with spastic cerebral palsy.Methods:A total of 42 patients with spastic cerebral palsy were included in this study,subjected to orthopedic surgery of the lower extremity,and immobilized in plaster cast for 4 to 6 weeks.The patients were divided into the treatment group and the control group according to the odd⁃even order of their visits(n=21 each).The treatment group was given floating acupuncture therapy plus conventional treatment,while the control group was given conventional treatment alone.Visual analogue scale(VAS)was used to record the severity of ankle pain on days 1,5 and 10 after removal of the plaster cast.Young’s modulus of elasticity(E)of the gastrocnemius on days 1,5 and 10 after removal of the plaster cast was measured by shear wave elastography.All data were subjected to statistical analysis.Results:There was no statistical difference in baseline data between the two groups(P>0.05).The VAS scores differed significantly either within or between groups on days 1,5 and 10 after removal of the plaster cast(P<0.05).The E values of gastrocnemius differed significantly either within or between groups on day 1 and 10 after removal of the plaster cast(P<0.05).Conclusion:Floating acupuncture therapy offers satisfactory analgesic effect on joint pain after orthopedic surgery of lower extremity in patients with spastic cerebral palsy.As shown by shear wave elastography,floating acupuncture therapy can more effectively reduce the gastrocnemius stiffness,relieve muscle spasm,and is clinically conducive to the postoperative rehabilitation.
