二维联合三维超声诊断胎儿颅脑畸形的价值 被引量:2

Value of two-dimensional combined with three-dimensional ultrasound in diagnosis of fetal craniocerebral malformation

摘要 目的分析二维联合三维超声诊断胎儿颅脑畸形的应用价值。方法抽取2018年3月至2021年3月濮阳市妇幼保健院收治的疑似胎儿颅脑畸形的孕妇82例,均行二维超声、三维超声诊断。以引产或产后诊断结果为"金标准",统计二维超声、三维超声单独诊断及二者联合诊断疑似胎儿颅脑畸形孕妇的诊断结果与诊断效能,并对胎儿不同颅脑畸形检出情况进行统计。结果本组82例疑似胎儿颅脑畸形孕妇,引产或产后确诊胎儿颅脑畸形阳性67例,阴性15例;采用二维超声联合三维超声诊断出真阳性66例,真阴性12例;采用二维超声诊断出真阳性61例,真阴性5例;采用三维超声诊断出真阳性63例,真阴性8例。二维超声联合三维超声诊断特异度(80.00%,12/15)高于二维超声(33.33%,5/15)及三维超声(53.33%,8/15),误诊率(20.00%,3/15)低于二维超声(66.67%,10/15)及三维超声(46.67%,7/15),P<0.05。二维超声、三维超声单独与联合诊断不同颅脑畸形检出率比较差异未见统计学意义(P>0.05)。结论二维联合三维超声诊断胎儿颅脑畸形的诊断价值较高,可为其早期筛查提供丰富科学依据。 Objective To analyze the value of two-dimensional combined with three-dimensional ultrasound in the diagnosis of fetal craniocerebral malformation.Methods A total of 82 pregnant women with suspected fetal craniocerebral malformation treated in Puyang Maternal and Child Health Hospital from March 2018 to March 2021 were selected,and they were diagnosed by two-dimensional ultrasound and three-dimensional ultrasound.Taking induced labor or postpartum outcome as the gold standard of diagnosis,the diagnostic results and diagnostic efficacy of two-dimensional ultrasonography,three-dimensional ultrasonography,and combination of them in the diagnosis of suspected fetal craniocerebral malformations in pregnant women were statistically analyzed,and the detection results of different fetal craniocerebral malformations were statistically analyzed.Results Among 82 pregnant women with suspected fetal craniocerebral malformations in the study,67 cases were positive fetal craniocerebral malformations,and 15 cases were negative after induction or delivery.The combination of two-dimensional and three-dimensional ultrasound showed that 66 cases were identified as true positive,and 12 cases as true negative;while 61 cases were identified as true positive and 5 cases as true negative by two-dimensional ultrasound;and 63 cases were identified as true positive and 8 cases as true negative by three-dimensional ultrasound.The diagnostic specificity of two-dimensional ultrasound combined with three-dimensional ultrasound(80.00%,12/15)was higher than that of two-dimensional ultrasound(33.33%,5/15)and three-dimensional ultrasound(53.33%,8/15);the misdiagnosis rate of two-dimensional ultrasound combined with three-dimensional ultrasound(20.00%,3/15)was lower than that of two-dimensional ultrasound(66.67%,10/15)and three-dimensional ultrasound(46.67%,7/15),P<0.05.There was no significant difference in the detection rate of different craniocerebral malformations among two-dimensional,three-dimensional ultrasound,and combination of them(P>0.05)
