摘要
目的:对比SLIPA喉罩全麻与气管插管全麻在妇科腹腔镜手术中的临床麻醉效果。方法:选择2018年2月~2020年2月在大连医科大学附属大连妇幼保健院接受妇科腹腔镜手术治疗的88例患者为研究对象,依据随机数字表法,随机将研究对象分为对照组和试验组(每组44例),对照组采取气管插管全麻方式,试验组应用SLIPA喉罩全麻,比较两组患者的血流动力学指标和不良反应发生率。结果:两组患者不同时间节点[插管(罩)即刻、插管(罩)后5min、拔管(罩)后即刻、拔管(罩)后5min]的MAP、HR均有波动,但试验组较为平稳,且均低于对照组,P<0.05;试验组患者的不良反应发生率为2.27%,低于对照组(13.64%),P<0.05。试验组各个阶段的SBP、DBP稳定值优于对照组,差异有统计学意义(P<0.05)。结论:在妇科腹腔镜手术的临床麻醉中,SLIPA喉罩全麻较气管插管全麻安全性更高,能够保证患者血流动力学指标平稳。
Objective:To compare the clinical anesthesia effect of general anesthesia by SLIPA laryngeal mask and general anesthesia by tracheal intubation in gynecological laparoscopic surgery.Methods:A total of 88 patients who received gynecological laparoscopic surgery in Dalian Maternal and Child Health Hospital affiliated to Dalian Medical University during from February 2018 to February 2020 were selected as the study objects.According to the random number table method,the study objects were randomly divided into two groups:control group and experimental group(44 cases in each group).The control group was given general anesthesia by tracheal intubation,and the experimental group was given general anesthesia by SLIPA laryngeal mask.The hemodynamic indexes and the incidence of adverse reactions were compared between the two groups.Results:The MAP and HR of the two groups of patients at different time nodes[immediately intubation(mask),5 minutes after intubation(mask),immediately after extubation(mask),and 5 minutes after extubation(mask)]fluctuated,however,the experimental group was relatively stable,and both were lower than the control group,P<0.05;the adverse reaction rate of patients in the experimental group was 2.27%,which was lower than the control group(13.64%),P<0.05.The stable values of SBP and DBP in each stage of the experimental group were better than those of the control group,and the difference was statistically significant(P<0.05).Conclusion:In the clinical anesthesia of gynecological laparoscopic surgery,the safety of general anesthesia by SLIPA laryngeal mask is higher than that of general anesthesia by tracheal intubation,and it can ensure the stability of hemodynamic indexes of patients.
