摘要
目的探究听力学检查在急性低频感音神经性听力损失中的临床意义。方法选择2015年3月~2020年2月我院耳鼻咽喉头颈外科收治的100例急性低频感音神经性听力损失(acute low-tone sensorineural hearing loss,ALHL)患者100例(104耳)为研究组,选择同期我院无听力相关疾病的听力正常者50例(50耳)为对照组,探究两组听力学检查的差异,以及在急性低频感音神经性听力损失中的临床意义。结果研究组患者中初次发病以30~40岁居多,且多为单侧发病,多数患者伴有耳鸣和耳闷等症状;在0.125、0.25、0.5及1 kHz的低频纯音听阈频率均显著高于对照组(P<0.05),但在2、3、4 kHz的高频纯音听阈频率与对照组间无显著差异(P>0.05);两组患者ABR均可引出正常波形,各波形潜伏期、波间期均处于正常范围;研究组在0.553、0.783、1.105和2.211 kHz的DP-gram幅值和信噪比(signal-noise ratio,SNR)值均显著高于对照组(P<0.05),但在4.416、6.250和8.837 kHz上,SNR值显著低于对照组(P<0.05),DP-gram幅值无显著差异(P>0.05);研究组-SP幅值、-SP/AP比值均显著高于对照组(P<0.05),但AP幅值和AP潜伏期与对照组无显著差异(P>0.05)。结论纯音听阈、ABR、声导抗、畸变产物耳声发射和耳蜗电图在急性低频感音神经性听力损失中具有较高的诊断价值。
Objective To explore the clinical significance of various audiological examinations in acute low-frequency sensorineural hearing loss.Methods 100 cases(104 ears)of acute low-frequency sensorineural hearing loss(ALHL)admitted to the Department of Otolaryngology Head and Neck Surgery of our hospital from March 2015 to February 2020 were selected as the study group.50 normal hearing patients(50 ears)without hearing-related diseases tested in our hospital during the same period were selected as the control group.The differences between the two groups were studied to explore the audiological examination in clinical significance in acute low frequency sensorineural hearing loss.Results Most of the 100 ALHL patients in our hospital were 30-40 years old,and most were unilateral,and accompanied by tinnitus and stuffy ear.The pure tone hearing thresholds at 0.125 kHz,0.25 kHz,0.5 kHz and 1 kHz in the study group were significantly higher than that in the control group,and the difference was statistically significant(P<0.05).However,there was no statistically significant difference between the pure tone hearing thresholds at 2,3 and 4 kHz in the study group and the control group(P>0.05).The Patients in the both groups were tested by ABR,which were within the normal range.The DP-gram amplitude and SNR values at 0.553,0.783,1.105 and 2.211 kHz in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).However,at 4.416,6.250 and 8.837 kHz,the SNR value of the study group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05),while the difference in the DP-gram amplitude was not statistically significant(P>0.05).The-sp amplitude and-sp/AP ratio of the study group were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).However,the differences in the amplitude and the latency of AP were not statistically significant(P>0.05).Conclusion Pure
