摘要
目的研究新生儿高胆红素血症(NHB)血清总胆红素(TBIL)及8-异前列腺素F2α(8-iso-PGF2α)水平与血肌酐(Scr)、β_(2)微球蛋白(β_(2)-MG)、血尿素氮(BUN)和超氧化物歧化酶(SOD)变化的关系。方法分别选取2017年4月至2021年4月我院收治的NHB患儿104例(NHB组)和健康新生儿107例(对照组)为研究对象进行横断面研究,分别采集性别、日龄、分娩方式和出生体重等基本信息,于入组第1 d采集外周静脉血5 mL并检测血清TBIL、8-iso-PGF2α、Scr、β_(2)-MG、BUN和SOD水平,比较两组检测结果差异并分析其与病情严重程度和肾功能损伤的关系。结果NHB组血清TBIL、8-iso-PGF2α、Scr、β_(2)-MG和BUN水平高于对照组,血清SOD水平低于对照组,差异有统计学意义(P<0.05);随着病情严重程度增加,NHB患儿血清TBIL、8-iso-PGF2α和β_(2)-MG呈上升趋势(P<0.05),血清SOD水平呈下降趋势,各组差异均有统计学意义(P<0.05);NHB患儿合并肾损伤者血清8-iso-PGF2α、Scr、β_(2)-MG和BUN水平高于未合并肾损伤者,血清SOD水平低于未合并肾损伤者,差异有统计学意义(P<0.05);NHB患儿血清8-iso-PGF2α水平与Scr、β_(2)-MG和BUN水平呈明显正相关性(P<0.05),与血清SOD水平呈明显负相关性。结论NHB患儿血清TBIL和8-iso-PGF2α水平均明显升高,其中8-iso-PGF2α与SOD具有明显相关性,可反映机体氧化应激水平,并为NHB评估病情严重程度和肾功能损害提供参考依据。
Objective To study the relationship between levels of serum total bilirubin(TBIL)and 8-iso-prostaglandin F2α(8-iso-PGF2α)and changes of serum creatinine(Scr),β_(2)-microglobulin(β_(2)-MG),blood urea nitrogen(BUN)and superoxide dismutase(SOD)in neonatal hyperbilirubinemia(NHB).Methods 104 children with NHB(NHB group)and 107 healthy neonates(control group)treated in the hospital between April,2017 and April,2021 were selected as samples for a cross-sectional study.Basic information,such as gender,age in days,delivery methods and birth weight,were collected.On the 1 st d of enrollment,5 mL of peripheral venous blood were collected from subjects to detect levels of serum TBIL,8-iso-PGF2α,Scr,β_(2)-MG,BUN and SOD.The detection results were compared between the two groups and their relationships with disease severity and renal function impairment were evaluated.Results Levels of serum TBIL,8-iso-PGF2α,Scr,β_(2)-MG and BUN in the NHB group were higher than those in the control group,while the level of serum SOD was lower than that in the control group(P<0.05).As the disease severity increased,levels of serum TBIL,8-iso-PGF2αandβ_(2)-MG showed upward trends,while the level of serum SOD showed a downward trend(P<0.05),and the differences among the groups were statistically significant(P<0.05).Levels of serum 8-iso-PGF2α,Scr,β_(2)-MG and BUN in children with NHB complicated with renal injury were higher than those of children without renal injury,while the level of serum SOD was lower than that of children without renal injury(P<0.05).Serum 8-iso-PGF2αlevel in children with NHB was significantly positively correlated with levels of Scr,β_(2)-MG and BUN,and was significantly negatively correlated with serum SOD level(P<0.05).Conclusion Levels of serum TBIL and 8-iso-PGF2αare significantly increased in children with NHB.8-iso-PGF2αis significantly correlated with SOD,and can reflect the body’s oxidative stress level and provide a reference for evaluating the severity of NHB and renal function impairment.
