摘要
目的 评价ω-3多不饱和脂肪酸(ω-3 PUFAs)应用于多囊卵巢综合征(PCOS)患者的疗效。方法 计算机检索PubMed、Medline、Web of Science、The Cochrane Library、EMBASE、中国知网、万方与维普数据库,纳入从建库至2020年1月31日间、关于ω-3 PUFAs应用于PCOS患者的随机对照试验。由2位评价员独立筛选文献、提取资料和评价纳入研究的偏倚风险,用RevMan 5.4和Stata14.0软件对纳入研究进行Meta分析。结果 本Meta分析共纳入15项研究。Meta分析结果显示,相较于对照组,ω-3 PUFAs显著降低了患者甘油三酯(TG)[MD=-14.69,95%CI(-20.35,-9.04),P<0.01]、胰岛素抵抗指数(HOMA-IR)[MD=-0.50,95%CI(-0.68,-0.31),P<0.01]、硫酸脱氢表雄酮(DHEAS)[MD=-0.26,95%CI(-0.69,0.18),P<0.01]水平;但对患者体质量指数(BMI)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、超敏C反应蛋白(hs-CRP)、性激素结合球蛋白(SHGB)、总睾酮(TT)、黄体生成素(LH)、卵泡刺激素(FSH)、多毛症评分(mF-G)水平无显著影响(P>0.05)。结论 ω-3 PUFAs应用于PCOS患者有一定的疗效,但尚需设计更严谨的临床对照研究加以验证。
Objective:To evaluate the efficacy of ω-3 polyunsaturated fatty acids(ω-3 PUFAs) on polycystic ovary syndrome(PCOS).Methods:The randomized controlled trials(RCT) of ω-3 PUFAs in PCOS patients were searched from databases including PubMed, Medline, Web of Science, Cochrane Library, EMBASE,CNKI,Wanfang and VIP from build database to January 31,2020. The literature, extracted data were screened and the risk of bias included in the study was evaluated by two evaluators independently. Meta-analysis was performed with RevMan 5.4 and Stata 14.0 software by two independent researchers.Results:Fifteen RCT studies were included in the meta-analysis. The results of meta-analysis showed that ω-3 PUFAs significantly reduced plasma triglyceride[MD=-14.69,95%CI(-20.35,-9.04),P<0.01],insulin resistance index[MD=-0.50,95%CI(-0.68,-0.31),P<0.01],dehydroepiandrosterone sulfate(DHEAS)[MD=-0.26,95%CI(-0.69,0.18),P<0.01],compared with the control group. However, there was no significant difference in BMI,plasma total cholesterol(TC),high-density lipoprotein(HDL-C),low-density lipoprotein(LDL-C),high sensitivity C-reactive protein(hs-CRP),sex hormone-binding globulin(SHBG),total testosterone(TT),LH,FSH and Ferriman-Gallwey(mF-G) scores(P>0.05).Conclusions:ω-3 PUFAs have a considerable effect on the treatment of PCOS. However, more rigorous clinical control studies are needed to verify it.
