摘要
目的研究儿童ALL早期治疗反应及循环血内骨桥蛋白、铁调素含量对预后的预测价值。方法选取92例急性淋巴细胞白血病(acute lymphocytic leukemia,ALL)患儿作为实验组,根据患儿2年后预后情况分为预后较差组(n=20)和预后良好组(n=72),同时选取89例同期健康体检小儿作为对照组(n=89)。对比预后较差组和预后良好组患儿早期治疗反应;对比3组小儿循环血内骨桥蛋白、铁调素含量;绘制ROC曲线分析循环血内骨桥蛋白和铁调素含量对预后的预测价值。结果预后良好组患儿PGR、M1和MRD水平<10^(-4)人数占比显著高于预后较差组患儿(P<0.05)。预后较差组和预后良好组小儿循环血内骨桥蛋白、铁调素含量显著高于对照组小儿(P<0.05);预后较差组小儿循环血内骨桥蛋白、铁调素含量显著高于预后良好组小儿(P<0.05)。结论早期治疗反应及循环血内骨桥蛋白、铁调素含量在儿童ALL中发挥着重要的作用,对预后具有较高的预测价值。
Objective To investigate early treatment response and prognostic value of circulating osteopontin and hepcidin levels in children with acute lymphocytic leukemia(ALL).Methods 92 children with ALL were enrolled set as the experimental group.THE experimental group was further divided into 2 subgroups according to the 2-year prognosis,poor prognosis group(n=20)and good prognosis group(n=72),meantime,another 89 healthy children were set as the control group.The treatment response rate was compared between good prognosis group and poor prognosis group.The levels of circulating osteopontin and hepcidin were compared among the 3 groups.Then receiver operating characteristic(ROC)curve was drawn to analyze the prognostic value of circulating blood osteopontin and calmodulin.Results The proportion of prednisone good response(PGR),M1 and minimal residual disease(MRD)<10^(-4) were significantly higher in good prognosis group than in poor prognosis group(P<0.05).Levels of circulating osteopontin and hepcidin of poor prognosis group and good prognosis group were significantly higher than those of control group(P<0.05).Levels of circulating osteopontin and hepcidin of poor prognosis group were significantly higher than those of good prognosis group(P<0.05).Conclusion Early treatment response and the contents of circulating osteopontin and hepcidin play an important role in children with ALL,and have high predictive value for prognosis.
