摘要
目的探讨采用单侧弯角椎体成形术(PCVP)与双侧穿刺经皮椎体成形术(PVP)治疗胸腰椎骨质疏松性椎体压缩骨折的效果。方法选取2019年9月至2021年12月遂平县中医院收治的72例胸腰椎骨质疏松性椎体压缩骨折患者,按照随机数字表法分为观察组、对照组,每组36例。观察组接受PCVP治疗,对照组接受PVP治疗。记录两组患者临床指标(手术时间、术中出血量、水泥注入量),记录术前、术后24 h及术后7 d的疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI),记录两组并发症发生情况。结果观察组手术时间短于对照组,术中出血量、骨水泥注入量少于对照组(P<0.05)。两组不同时间点VAS评分、ODI指数比较,差异有统计学意义(P<0.05)。术前,两组VAS评分、ODI指数比较,差异无统计学意义(P>0.05);术后24 h及术后7 d,观察组VAS评分、ODI指数低于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论单侧PCVP术与双侧PVP术治疗胸腰椎骨质疏松性椎体压缩骨折均有较好效果,但PCVP手术时间短,术后疼痛改善以及功能恢复更快,并发症发生率更低。
Objective To explore the effect of unilateral percutaneous curved vertebro plasty(PCVP)and bilateral percutaneous vertebro plasty(PVP)in the treatment of thoracolumbar osteoporotic vertebral compression fractures.Methods A total of 72 patients with thoracolumbar osteoporotic vertebral compression fractures treated in Suiping Hospital of Traditional Chinese Medicine from September 2019 to December 2021 were randomly divided into observation group and control group,with 36 cases in each group.The observation group accepted with PCVP and the control group accepted with PVP.The clinical indexes(operation time,intraoperative bleeding and cement injection)were recorded,the visual analogue scale(VAS),Oswestry dysfunction index(ODI)24 hours and 7 days after operation were recorded,and the complications of the two groups were recorded.Results The operation time in observation group was shorter than that in control group,the intraoperative bleeding and cement injection in observation group were less than those in control group(P<0.05).There were statistical differences in VAS score and ODI index between two groups at different time points(P<0.05).There was no statistical difference in VAS score and ODI index between the two groups before operation(P>0.05).24 hours and 7 days after operation,the VAS score and ODI index in observation group were lower than those in control group(P<0.05).The incidence rate of complications in observation group was lower than that in control group(P<0.05).Conclusion Both unilateral PCVP and bilateral PVP are effective in the treatment of thoracolumbar osteoporotic vertebral compression fractures,but PCVP has the advantages of short operation time,improved postoperative pain,faster functional recovery and lower incidence of complications.
