摘要
目的探讨血清微小RNA223(miR-223)、微小RNA155(miR-155)检验对小儿肺炎支原体感染的诊断价值,为其临床诊治提供参考。方法选择福州市第二医院2019年2月至2020年12月收治的100例小儿肺炎支原体感染患儿(观察组)作为研究对象,另取同期体检的50名健康儿童作为对照组。比较两组受试者血清降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素6(IL-6)及miR-223、miR-155水平,采用Pearson法对血清miR-223、miR-155水平与PCT、CRP、IL-6水平的相关性进行分析,并通过受试者工作特征(ROC)曲线分析血清miR-223、miR-155检验对小儿肺炎支原体感染的诊断价值。结果观察组患儿血清miR-223、miR-155表达及PCT、CRP、IL-6水平均明显高于对照组,差异有统计学意义(P<0.05)。经Pearson相关性分析,小儿肺炎支原体感染患者血清miR-223、miR-155水平与血清PCT、CRP、IL-6呈正相关(P<0.05)。经ROC分析显示,miR-223+miR-155的AUC为0.926,明显高于miR-223和miR-155(P<0.05);且miR-223与miR-155的截断值分别为11.29、4.35。结论miR-223与miR-155在小儿肺炎支原体感染患者中均呈现高表达,且与血清PCT、CRP、IL-6水平呈正相关,对小儿肺炎支原体感染具有较高的临床诊断价值,初步证实了miR-223与miR-155具有成为预防小儿肺炎支原体肺炎新靶点的潜力。
Objective To explore the diagnostic value of serum microRNA223(miR-223)and microRNA155(miR-155)tests in children with Mycoplasma pneumoniae infection,so as to provide reference for its clinical diagnosis and treatment.Methods 100 children with Mycoplasma pneumoniae infection admitted to The Second Hospital of Fuzhou City from February 2019 to December 2020 were selected as research objects(observation group),and 50 healthy children who came to hospital for physical examination during the same period were selected as control group.And then,the serum levels of procalcitonin(PCT),C-reactive protein(CRP),interleukin 6(IL-6),miR-223 and miR-155 were compared between the two groups.Pearson method was used to analyze correlation between serum miR-223,miR-155 levels and PCT,CRP,IL-6 levels.And the diagnostic value of serum miR-223 and miR-155 in children with Mycoplasma pneumoniae infection was analyzed by receiver operating characteristic(ROC)curve.Results The expression of serum miR-223,miR-155 and the levels of PCT,CRP and IL-6 in the observation group were significantly higher than those in the control group,and difference was statistically significant(P<0.05).Pearson correlation analysis showed that serum miR-223 and miR-155 levels in children with Mycoplasma pneumoniae infection were positively correlated with serum PCT,CRP,and IL-6(P<0.05).ROC analysis showed that the AUC of miR-223+miR-155 was 0.926,significantly higher than that of miR-223 and miR-155(P<0.05).And the cutoff values of miR-223 and miR-155 were 11.29 and 4.35,respectively.Conclusion Both miR-223 and miR-155 are highly expressed in children with Mycoplasma pneumoniae infection,and are positively correlated with serum PCT,CRP and IL-6 levels,they have high clinical diagnostic values for children with Mycoplasma pneumoniae infection,which have been preliminarily confirmed to have the potential to become new targets for the prevention of MPP.
