摘要
目的 探讨竖脊肌平面阻滞对肾结石病人术后痛敏反应及伤害应激反应的影响。方法 前瞻性选取我院收治的肾结石病人102例,根据麻醉方式的不同将其分为对照组和研究组,每组各51例,对照组采用全麻,研究组在全麻基础上给予竖脊肌平面阻滞。比较两组临床指标、痛敏反应及心理应激指标。结果 研究组临床指标均优于对照组,差异有统计学意义(P<0.05);术后1、3、5天两组的VAS评分均有所下降,且研究组优于对照组,差异有统计学意义(P<0.05)。研究组手术应激指数、脑状态疼痛指数、舒适指数、焦虑指数及紧张指数等心理应激指标均优于对照组,差异有统计学意义(P<0.05)。结论 经皮肾镜取石采用竖脊肌平面阻滞结合全身麻醉,能有效改善临床指标,抗痛敏反应和应激反应好。
Objective To investigate the effect of erector spinae plane block on postoperative hyperalgesia and injury stress in patients with kidney stones.Methods 102 patients with renal calculi treated in our hospital were prospectively selected as the research object.According to different anesthesia methods, they were divided into control group and study group, with 51 cases in each group.The control group was given general anesthesia, and the study group was given vertical spinal muscle plane block on the basis of general anesthesia.The clinical indexes, pain sensitivity and psychological stress indexes were compared.Results The clinical indexes in the study group were significantly lower than those in the control group(P<0.05);The VAS score of the study group was lower than that of the control group(P<0.05).The psychological stress indexes such as surgical stress index(SSI),brain state pain index(PI),comfort index(CFI),anxiety index(Anxi)and tension index(NVI)in the study group were significantly lower than those in the control group(P<0.05).Conclusion In the treatment of percutaneous nephroscope holmium laser lithotripsy, the effect of vertical spinal muscle plane block combined with general anesthesia is remarkable, which can effectively improve clinical indexes, have good anti pain sensitivity and stress response.
