摘要
目的分析对比跗骨窦入路与传统L形入路治疗SendersⅡ、Ⅲ型跟骨骨折的疗效。方法选取2016年7月~2020年7月医院收治的75例SendersⅡ、Ⅲ型跟骨骨折患者,根据手术方法分为2组,对照组采用传统L形入路(n=36),观察组采用跗骨窦入路(n=39),2组术后均随访6个月。比较2组患者手术时间、术中出血量、骨折愈合时间,Maryland足关节功能评分和美国足踝外科协会(AOFAS)评分,临床疗效、影像学指标和并发症。结果与对照组相比,观察组手术时间更短(P<0.05),术中出血量更少(P<0.05);术后6个月2组患者Maryland评分和AOFAS评分比较,差异均无统计学意义(P>0.05);术后6个月2组总有效率比较,差异无统计学意义(P>0.05);术后6个月,2组患者Bohler角和Gissane角与术前比较均增加(P<0.05),2组间比较,差异均无统计学意义(P>0.05);观察组总并发症发生率低于对照组(P<0.05)。结论跗骨窦入路用于治疗SendersⅡ、Ⅲ型跟骨骨折能够取得与传统L形入路治疗同样的疗效,同时能够缩短手术时间,减少出血量和并发症。
Objective To analyze and compare the curative effect of SendersⅡandⅢfracture of calcaneus with tarsal sinus approach and traditional L-shaped approach.Methods Seventy-five patients with SendersⅡandⅢfracture of calcaneus admitted to the hospital from July 2016 to July 2020 were selected,according to the surgical method,they were divided into two groups,the control group used the traditional L-shaped approach(n=36),and the observation group used the tarsal sinus approach(n=39),and patients in both groups were followed up for 6 months after operation.The operation time,intraoperative blood loss,fracture healing time,Maryland foot joint function score and the American Orthopaedic Foot and Ankle Society(AOFAS)score,clinical efficacy,imaging indicators,and complications during follow-up were compared between the two groups.Results Compared with the control group,the observation group had shorter operation time(P<0.05),and less intraoperative blood loss(P<0.05).There were no significant differences in Maryland score and AOFAS score between the two groups at 6 months after operation(P>0.05).There were no statistically significant differences in the total effective rate between the two groups at 6 months after operation(P>0.05).Compared with preoperative,the Bohler angle and Gissane angle of the two groups were increased in 6 months after the operation(P<0.05),there was no significant difference between the two groups(P>0.05).Compared with the control group,the observation group had lower overall complication rate(P<0.05).Conclusion The tarsal sinus approach for the treatment of SendersⅡandⅢfracture of calcaneus can achieve the same curative effect as the traditional L-shaped approach,at the same time,it can shorten operation time,it has less bleeding and complications.
