摘要
目的:探讨Masquelet技术与骨移植治疗感染性骨缺损的效果及其对患者关节功能、白细胞计数(white blood cell count,WBC)、血沉、C反应蛋白(C-reactive protein,CRP)及降钙素原(procalcitonin,PCT)水平的影响。方法:回顾性选取2018年5月-2020年7月武穴市第一人民医院诊治的64例下肢长骨感染性骨缺损患者的临床资料。按照治疗方法分为观察组与对照组,各32例。观察组予以Masquelet技术,对照组予以骨移植治疗。比较两组术前、术后3个月的膝、踝Lowa评分,术前、术后4周PCT、CRP、WBC及红细胞沉降率,并发症。结果:术后3个月,观察组膝、踝Lowa评分均高于对照组(P<0.05)。术后4周,观察组PCT、CRP、WBC均低于对照组,血沉慢于对照组(P<0.05)。观察组并发症发生率低于对照组,但差异无统计学意义(P>0.05)。结论:与骨移植术相比,Masquelet技术治疗效果更好,有利于提高关节功能,减慢血沉,降低WBC、CRP及PCT水平。
Objective:To explore the effects of Masquelet technique and bone transplantation in the treatment of infectious bone defects and its influence on patients’joint function,white blood cell count(WBC),erythrocyte sedimentation rate,C-reactive protein(CRP)and procalcitonin(PCT)levels.Method:The clinical data of 64 patients with infectious bone defects of lower extremity long bones treated in Wuxue First People’s Hospital from May 2018 to July 2020 were retrospectively selected.The patients were divided into observation group and control group according to treatment method,32 cases in each group.The observation group was given Masquelet technique and the control group was given bone transplantation treatment.Knee and ankle Lowa scores before and 3 months after surgery,PCT,CRP,WBC,erythrocyte sedimentation rate before and 4 weeks after surgery and complications were compared between the two groups.Result:Three months after surgery,knee and ankle Lowa scores in the observation group were higher than those in the control group(P<0.05).Four weeks after surgery,PCT,CRP and WBC in the observation group were lower than those in the control group,and erythrocyte sedimentation rate was slower than that in the control group(P<0.05).The incidence of complications in observation group was lower than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion:Compared with bone transplantation,the Masquelet technique has a better curative effect,which is beneficial to improve joint function,slow the erythrocyte sedimentation rate and reduce the levels of WBC,CRP and PCT.
