阶梯式营养干预对胃癌术后化疗患者化疗耐受性、免疫因子及不良反应的影响 被引量:18

摘要 目的研究阶梯式营养干预对胃癌术后化疗患者化疗耐受性、免疫因子及不良反应的影响。方法选择2018年7月至2021年6月于我院行手术治疗并行术后化疗的胃癌患者106例,依照随机数字表法分为试验组(n=53)与对照组(n=53)。两组均行CAPEOX方案化疗,对照组术后给予常规饮食干预,试验组给予常规饮食干预+阶梯式营养干预。观察两组患者化疗耐受性。两组干预前后血清前白蛋白、白蛋白、总蛋白等营养指标,血清CD3;、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)等免疫因子水平及不良反应。结果试验组患者化疗耐受性良好率大于对照组(P<0.05)。化疗后,试验组血清前白蛋白、白蛋白、总蛋白水平均高于对照组(P<0.05)。化疗后,试验组CD3;、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,CD8^(+)低于对照组(P<0.05)。试验组各不良反应Ⅲ~Ⅳ度发生率低于对照组(P<0.05)。结论阶梯式营养干预可为胃癌术后化疗患者化疗提供充足的营养,改善机体免疫,减少不良反应或降低其严重程度,提高化疗耐受性。 Objective To study the effects of stepped nutrition intervention on chemotherapy tolerance,immune factors and adverse reactions in patients with gastric cancer undergoing postoperative chemotherapy.Methods A total of 106 patients with gastric cancer who received surgical treatment and postoperative chemotherapy in our hospital from July 2018 to June 2021 were selected and divided into Experimental group(n=53)and Control group(n=53)according to random number table method.Both groups received CAPEOX regimen chemotherapy,the Control group was given routine diet intervention after surgery,and the Experimental group was given routine diet intervention+stepped nutrition intervention.The chemotherapy tolerance of two groups was observed.Serum pre-albumin,albumin,total protein and other nutritional indicators,serum CD3;,CD4^(+),CD8^(+)and CD4^(+)/CD8^(+),other immune factors before and after intervention in the two groups,and adverse reactions.Results The good rate of chemotherapy tolerance in Experimental group was higher than that in Control group(P<0.05).After chemotherapy,the serum levels of pre-albumin,albumin and total protein in Experimental groups were higher than those in Control group(P<0.05).After chemotherapy,the levels of CD3;,CD4^(+)and CD4^(+)/CD8^(+)in Experimental groups were higher than those in Control group,and CD8^(+)was lower than that in Control group(P<0.05).The incidence of grade III-IV ADR in Experimental group was lower than that in Control group(P<0.05).Conclusion Stepped nutrition intervention can provide adequate nutrition for postoperative chemotherapy for patients with gastric cancer,improve chemotherapy tolerance,body immunity,and reduce adverse reactions or their severity.
