摘要
目的:探讨急性脑梗死患者血清中miR-34b的表达特征,分析其与预后的关系。方法:收集急性脑梗死患者共87例为观察组,87例体检证实为健康的成人血清标本作为对照组,应用实时荧光定量PCR法检测血清中miR-34b的表达。结果:观察组中miR-34b的表达水平明显低于对照组[(1.06±0.25)与(1.32±0.34),P<0.01],CXCL12的表达水平明显高于对照组[(19.47±3.26)与(7.37±1.06),P<0.01]。血清中miR-34b表达下调对急性脑梗死诊断具有预测价值,CXCL12曲线下面积AUC为0.9880,95%置信区间(CI):0.9772~0.9988。血清中CXCL12表达上调对急性脑梗死诊断具有预测价值,CXCL12的预测价值要优于miR-34b。miR-34b在不同梗死灶最大径和有无高血压、高血脂患者的比较中差异有统计学意义(均P<0.05),而在不同性别、年龄、有无吸烟和饮酒史的比较中差异无统计学意义(均P>0.05)。相关回归分析显示miR-34b与格拉斯哥预后(GOS)评分具有正相关性(P<0.01),CXCL12与GOS评分具有负相关性(P<0.01)。miR-34b和CXCL12具有负相关性(P<0.01)。结论:急性脑梗死患者血清中miR-34b的表达下调,对病变形成和进展有一定促进作用,早期检测miR-34b对判断预后有一定价值。
Objective:To investigate the expression characteristics of miR-34b in serum of patients with acute cerebral infarction and analyze its relationship with prognosis.Methods:A total of 87 patients with acute cerebral infarction were collected as observation group and 87 healthy adult as control group.The expression of miR-34b in serum was detected by real-time fluorescence quantitative PCR.Results:The expression level of miR-34b in the observation group was significantly lower than that in the control group(1.06±0.25 vs.1.32±0.34,P<0.01),and the expression level of CXCL12 was significantly higher than that in the control group[(19.47±3.26)mg/ml vs.(7.37±1.06)mg/ml,P<0.01].Down-regulation of miR-34b expression in serum had predictive value for the diagnosis of acute cerebral infarction.The AUC of CXCL12 was 0.9880 with a 95%confidence interval(CI)of 0.9772 to 0.9988.The up-regulation of CXCL12 expression in serum had a predictive value for the diagnosis of acute cerebral infarction.The predictive value of CXCL12 was better than that of miR-34b.There were significant differences in the miR-34b between the maximum diameter of the infarct,the presence or absence of hypertension and hyperlipidemia(all P<0.05),but no significant differences between the sex,age,the presence or absence of smoking and alcohol consumption(all P>0.05).Correlation regression analysis showed a positive correlation between miR-34b and GOS score and a negative correlation between CXCL12 and GOS score(all P<0.01).There was a negative correlation between miR-34b and CXCL12(P<0.01).Conclusion:The down-regulation of miR-34b expression in serum of patients with acute cerebral infarction can promote the transformation and progression of the disease to a certain extent.Early detection of miR-34b has certain value in judging the prognosis.
