摘要
目的 探讨温针灸配合股直肌神经入肌点推拿点按疗法对中风偏瘫患者下肢运动功能障碍和生活质量的影响。方法 选择我院2019年1月至2021年1月收治的117例中风偏瘫患者作为研究对象,按照随机数字表法将其分为观察组(n=59)和对照组(n=58),在接受常规治疗和康复干预的基础上,对照组接受针刺治疗,观察组接受温针灸配合股直肌神经入肌点推拿点按疗法,连续治疗16周后,比较两组患者的下肢运动功能情况、股直肌平均肌电值(AEMG)、日常生活活动能力和生活质量。结果 治疗前,两组患者的ASIA运动功能评分和Fugl-Meyer下肢运动功能评分、股直肌AEMG、日常生活活动能力量表(ADL)、脑卒中专用生活质量量表(SS-QOL)评分比较,差异不具有统计学意义(P>0.05);治疗后,两组患者ASIA运动功能评分和Fugl-Meyer下肢运动功能评分、股直肌AEMG及ADL、SS-QOL评分显著提升,且观察组显著高于对照组,差异具有统计学意义(P<0.05)。结论 温针灸配合股直肌神经入肌点推拿点按疗法治疗中风偏瘫患者,能够明显改善其下肢运动功能、日常生活能力,提升生活质量。
Objective To explore the effect of warm acupuncture and moxibustion combined with massage and pointpressing therapy at rectus femoris nerve entry point on lower limb motor dysfunction and quality of life in patients with stroke hemiplegia.Methods A total of 117 patients with stroke hemiplegia admitted in our hospital from January 2019 to January 2021 were selected as the research objects.The patients were divided into observation group(n=59) and control group(n =58) according to the random number table method.On the basis of routine treatment and rehabilitation intervention,the control group was treated with acupuncture,and the observation group was treated with warm acupuncture and moxibustion combined with massage and point-pressing therapy at rectus femoris nerve entry point.After 16 weeks of continuous treatment,the lower limb motor function,average surface electromyography(AEMG) of rectus femoris,ability of daily living and quality of life were compared between the two groups.Results Before treatment,there were no significant differences in ASIA motor function score,Fugl-Meyer lower limb motor function score,AEMG of rectus femoris,activity of daily living scale(ADL) and stroke specific quality of life scale(SS-QOL) between the two groups(P>0.05);after treatment,the ASIA motor function score,Fugl-Meyer lower limb motor function score,AEMG of rectus femoris,ADL and SS-QOL scores of the two groups significantly increased,and those of the observation group were significantly higher than the control group,the differences were statistically significant(P<0.05).Conclusion Warm acupuncture and moxibustion combined with massage and point-pressing therapy at rectus femoris nerve entry point can significantly improve the lower limb motor function,daily living ability and quality of life of patients with stroke hemiplegia.
