摘要
目的:观察基于通筋宣痹法针推结合治疗肩周炎急性疼痛期的临床疗效。方法:选取急性疼痛期的肩周炎患者,按照患者选择意愿分为2组各30例。治疗组予针推结合通筋宣痹法治疗,每周3次,治疗2周;对照组予肩关节软组织封闭术治疗,每周1次,治疗2周。在治疗1周和治疗2周后分别通过Constant-Murley (CM)评分及疼痛视觉模拟评分法(VAS)评分对肩关节功能及疼痛情况进行观测,并比较2组间临床疗效。结果:因失访等原因最终纳入统计的患者共53例,治疗组25例,对照组28例。与治疗前比较,2组治疗2周后肩关节功能CM评分及疼痛VAS评分均有不同程度改善,差异均有统计学意义(P<0.05)。治疗1周后,治疗组在肩关节功能及疼痛的改善方面均不及对照组,差异均有统计学意义(P<0.05);但治疗2周后,2组肩关节功能CM评分及疼痛VAS评分组间比较,差异均无统计学意义(P>0.05)。临床总有效率治疗组76.00%,对照组85.71%,2组临床疗效比较,差异无统计学意义(P>0.05)。结论:基于通筋宣痹法针灸推拿结合治疗肩周炎急性疼痛期可以有效缓解肩周炎患者的疼痛,改善肩关节功能,临床疗效与封闭疗法相当。
Objective:To observe the clinical effect of the combination of acupuncture and Chinese massage based on the method of unblocking sinews and diffusing impediment on scapulohumeral periarthritis at acute pain stage. Methods:Patients with scapulohumeral periarthritis at acute pain stage were selected and divided into two groups,with 30 cases in each group. The treatment group was treated with acupuncture and Chinese massage based on the method of unblocking sinews and diffusing impediment three times a week for two weeks. The control group was treated with shoulder soft tissue closure once a week for two weeks. The shoulder function and pain were respectively observed by Constant Murley(CM) and Visual Analogue Scale(VAS) one week and two weeks after treatment,and the clinical effects were compared between the two groups. Results:A total of 53 patients were included in the statistics due to loss of follow-up,including 25 patients in the treatment group and 28 patients in the control group. Compared with those before treatment,the CM scores of shoulder joint function and VAS scores of pain in both groups were changed to varying degrees after two weeks of treatment,differences being significant(P<0.05). After one week of treatment,the improvement of shoulder function and pain in the treatment group was less significant than that in the control group,differences being significant(P<0.05). However,after two weeks of treatment,there was no significant difference in the scores of CM and VAS between the two groups(P>0.05).The total effective rate was 76.00% in the treatment group and 85.71% in the control group, there being no significant difference between the two groups(P>0.05). Conclusion:The combination of acupuncture and Chinese massage based on the method of unblocking sinews and diffusing impediment for scapulohumeral periarthritis at acute pain stage, can effectively relieve the pain of shoulder and improve the function of shoulder joint,whose clinical effect is equivalent to that of closed therapy.
