摘要
目的:观察白芍总苷胶囊联合羟氯喹和来氟米特治疗类风湿关节炎(RA)伴白细胞减少患者的效果。方法:选取60例RA伴白细胞减少患者为研究对象,按照随机数字表法分为观察组与对照组各30例。对照组采用硫酸羟氯喹片联合来氟米特片口服治疗,观察组在对照组基础上加用白芍总苷胶囊治疗,比较两组临床疗效、C反应蛋白(CRP)水平、类风湿因子(RF)水平、红细胞沉降率(ESR)、炎性因子[白细胞介素-17(IL-17)、白细胞介素-23(IL-23)、转化生长因子-β(TGF-β)]水平、视觉模拟评分法(VAS)评分和关节炎疾病活动性(DAS28)评分。结果:观察组治疗总有效率为93.33%,明显高于对照组的66.67%,差异有统计学意义(P<0.05);治疗后,两组CRP、RF、ESR水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组IL-17、IL-23、TGF-β水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组VAS评分和DAS28评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:白芍总苷胶囊联合羟氯喹、来氟米特治疗RA伴白细胞减少患者可提高治疗总有效率,以及降低VAS评分、DAS28评分、CRP水平、RF水平、ESR和炎性因子水平,效果优于羟氯喹联合来氟米特治疗。
Objective:To observe effects of total glucosides of paeonia capsules combined with Hydroxychloroquine and Leflunomide in treatment of patients with rheumatoid arthritis(RA)and leukopenia.Methods:60 patients with RA and leukopenia were selected as the research objects,and were divided into observation group and control group according to the random number table method,30 cases in each.The control group was treated with Hydroxychloroquine sulfate tablets combined with Leflunomide tablets orally,while the observation group was additionally treated with total glucosides of paeonia capsules on the basis of those of the control group.The clinical effects,the C-reactive protein(CRP)level,the rheumatoid factor(RF)level,the erythrocyte sedimentation rate(ESR),the inflammatory factor levels[interleukin-17(IL-17),interleukin-23(IL-23),transforming growth factor-β(TGF-β)],the visual analogue scale(VAS)score and the arthritis disease activity(DAS28)score were compared between the two groups.Results:The total effective rate of the observation group was 93.33%,which was significantly higher than 66.67%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of CRP,RF and ESR in the two groups were lower than those before the treatment;those of the observation group were lower than those of the control group;and the differences were statistically significant(P<0.05).After the treatment,the levels of IL-17,IL-23 and TGF-βin the two groups were lower than those before the treatment;those of the observation group were lower than those of the control group;the differences were statistically significant(P<0.05).Further,after the treatment,the VAS scores and the DAS28 scores of the two groups were lower than those before the treatment;those of the observation group were lower than those of the control group;and the differences were statistically significant(P<0.05).Conclusions:Total glucosides of paeony capsules combined with Hydroxychloroquine and Leflunomide in the treatment of
