摘要
目的研究糖尿病酮症患者体脂分布的特点以及内脏脂肪面积(VFA)、皮下脂肪面积(SFA)与相关代谢指标的相关性。方法选取2007年2月—2018年5月成都市某医院收治的糖尿病酮症患者46例为研究对象,收集患者的临床资料[性别、年龄、体质量指数(BMI)、血压、血脂、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、随机血糖、糖化血红蛋白(HbA1c)、尿酸、β-羟丁酸水平等]。利用腹部CT数据计算所有患者的VFA及SFA。采用Pearson相关分析VFA、SFA与HbA1c、血脂、尿酸、BMI等指标的相关性。结果46名糖尿病酮症患者中,1型糖尿病患者13例,2型糖尿病患者16例,糖尿病分型待定患者17例。患者平均VFA(97.92±59.10)cm^(2),平均SFA(144.50±78.26)cm^(2)。VFA、SFA分别与高密度脂蛋白胆固醇(HDL-C)呈负相关(r=-0.517,P<0.001;r=-0.392,P=0.007),与BMI呈正相关(r=0.823,P<0.001;r=0.734,P<0.001);VFA、SFA增加分别与随机血糖、收缩压、舒张压、HbA1c、TG、TC、LDL-C、尿酸无相关性(P>0.05)。结论糖尿病酮症患者VFA、SFA与HDL-C水平降低及BMI升高相关,提示人体脂肪堆积可能是导致代谢综合征、心血管事件的危险因素。
Objective To study the characteristics of body fat distribution and the correlation between visceral and subcutaneous fat content and related metabolic indexes in diabetic ketosis patients.Methods Forty-six patients with diabetic ketosis admitted to a hospital in Chengdu from February 2007 to May 2018 were selected for the study,and their general clinical data,including gender,age,body mass index(BMI),blood pressure and lipids(TG,TC,LDL-C),random blood glucose,glycosylated hemoglobin(HbA1c),uric acid,andβ-hydroxybutyric acid levels,were collected.Visceral fat area(VFA)and subcutaneous fat area(SFA)were calculated for all pa-tients using abdominal CT data.Pearson correlation analysis was used to analyze the correlation between VFA and SFA and HbA1c,blood lipids,uric acid,and BMI.Results The general data of 46 diabetic ketosis patients are shown in Table 1,including 13 patients with type 1 diabetes,16 patients with type 2 diabetes,and 17 pa-tients with undetermined diabetes.The mean VFA of patients was 97.92±59.10 cm^(2)and the mean SFA was 144.50±78.26 cm^(2).Patients'VFA and SFA were negatively correlated with HDL-C(r=-0.517,P<0.001;r=-0.392,P=0.007)and positively correlated with BMI(r=0.823,P<0.001;r=0.734,P<0.001).The VFA and SFA of patients were not correlated with their random blood glucose,systolic blood pressure,dias-tolic blood pressure,HbA1c,TG,TC,LDL-C,or uric acid(P>0.05).Conclusion VFA and SFA were asso-ciated with decreased HDL-C level and increased BMI in diabetic ketosis patients,suggesting that fat accumula-tion in the body may be a risk factor for metabolic syndrome and cardiovascular events.
